Primary Care
STOP ALL NSAIDS (non-steroidal anti-inflammatory drugs) 7 DAYS PRIOR TO SURGERY: i.e., Motrin, Advil, Ibuprofen, Aleve, Voltaren, Indocin, Naprosyn, Celebrex, Arthrotec, Mobic. ALL Blood Thinners need to be stopped 5-10 days prior to surgery. If you take any form of blood thinner (Warfarin/Coumadin) or anti-platelet drug (Plavix, Pradaxa, Xarelto, etc) please check with your prescribing doctor to see when you should stop this medication prior to surgery. Report to the main lobby of the hospital (Admissions Department) the day of your surgery at the time designated by our office. Please bring your insurance cards. Our office will call your insurance carrier to pre-authorize your surgery, and request a referral from your primary care physician (if HMO).

Multiple Sclerosis
In general, Neuromodulation involves interfacing the nervous system with circuits or implanting catheters to deliver medication directly. This allows for direct treatment of a variety of neurologic problems including pain, spasticity from spinal cord injury, brain injury or multiple sclerosis, movement disorders and others. Neuro-modulation is a method of altering signaling of the nervous system to treat symptoms of a neurological disease or disorder. Conditions that are treated include a variety of movement disorders: Parkinson’s Disease, essential tremor, dystonia and Tourette’s Syndrome as well as some forms of chronic pain. Using implanted circuits, the symptoms of these disorders are diminished or in some cases completely alleviated. These implanted devices use electrical signals that can be controlled by the patient. Neuromodulation includes all forms of neurostimulation including Deep Brain Stimulation, Spinal Cord Stimulation, Peripheral Nerve Stimulation and Motor Cortex Stimulation as well as spinal pump delivery systems to treat spasticity. Neuro-modulation uses the most sophisticated technology to treat a variety of symptoms and Washington Brain & Spine Institute is seen as a leader in the field. Dr. Zachary Levine leads the team. He works with an experienced group of electrophysiologists and neurologists as well as pain management physicians to achieve the best results possible for our patients. The emphasis is on patient outcomes using a team approach.
Epilepsy
Epilepsy is most commonly treated with medication. However, in some circumstances surgery is needed to eliminate the focus of seizures or a device may be implanted to decrease the number of seizures. Dr. Levine works with epileptologists and epilepsy monitoring unit at Holy Cross Hospital to help screen and eventually treat surgically those patients that require this approach. Our practice was one of the first in our region to use laser ablation to treat epilepsy. This new approach allows for surgical treatment of epilepsy using a minimally invasive technique. Through a single incision, less than one inch long, we can eliminate the seizure focus. With this new technique the patient usually goes home the following day.

Ultrasound
MRgFUS or HIFU is a form of treatment for tremor and symptoms of Parkinson’s Disease. Essential tremor, also referred to as familial, idiopathic or benign tremor, causes uncontrollable shaking that can impact a person’s ability to live an independent and active lifestyle. When an essential tremor patient does not get tremor relief from medications, the MR Guided Focused Ultrasound treatment could be an option. Parkinson’s Disease may present with predominantly tremor that is not well treated with medication. MRgFUS is also effective at treating this form of tremor and is approved for the treatment of dyskinesia associated with Parkinson’s Disease.

MR Guided Focused Ultrasound uses targeted ultrasound waves to treat the small spot in the brain considered to be responsible for causing tremor. Wayne J. Olan, MD is board-certified in Radiology, and is the Director of Interventional and Endovascular Neurosurgery, and an associate professor of Neurosurgery and Radiology at The George Washington University School of Medicine & Health Sciences. He also serves as Co-director of the Endovascular Stroke Program at GW. He completed his training in Radiology in New York and fellowship training in Neuroradiology with subspecialty training in Interventional Neuroradiology at GW.

Chemotherapy
Open surgery also allows the use of other treatment during surgery. Chemotherapy and radiation can be delivered directly to the tumor during an open operation. This allows for higher concentrations of drug or radiation to be delivered to the site without exposing the rest of the body. Tumors at the base of the skull often require more extensive procedures and more lengthy operations. These tumors often involve the nerves and vessels that exit and enter the skull. Special procedures and monitoring are used during the surgery to increase the safety of the patient.
Radiation Therapy
Neurosurgery is the practice of medicine which deals with diseases of the nervous system, including the spine and spinal cord, brain and peripheral nerves. The surgical treatment of these diseases requires the skill and expertise of a surgeon who is specially trained - hence the term neurosurgeon. A neurosurgeon is not just a "brain surgeon", but treats patients with many different illnesses. We perform brain tumor surgery and lumbar, thoracic and cervical spine surgery among many other surgical procedures. The majority of the patients treated by neurosurgeons include those with: back or leg pain; neck or arm pain; strokes; tumors of the brain, spine or spinal cord; trauma to the brain, spine or spinal cord; abnormalities of blood vessels within the brain, spine or spinal cord; and abnormalities of nerves throughout the body. Although neurosurgery is by nature a surgical field, many patients with neurological illnesses which do not require surgery often seek the opinion of a neurosurgeon to discuss non-surgical or minimally invasive treatments for their problems. Some of these minimally invasive and non-surgical treatments include: endoscopic spine and brain surgery, computer assisted stereotactic surgery, stereotactic radiosurgery (i.e., using radiation therapy to treat lesions of the nervous system), and interventional radiology (i.e., treating lesions in the blood vessels of the nervous system). Minimally Invasive Surgery
Most of the surgical procedures for the spine are for decompression of the spinal nerves or spinal cord. The compression can cause pain, weakness and sensory changes that can be reversible with decompression. Surgery is not always the first choice for treatment, but it may be necessary in certain circumstances, Minimally invasive approaches are used, when possible to minimize postoperative discomfort and hasten recovery. The acronym MIS indicates Minimally Invasive Surgery. Listed below are some of the more common procedure done. This is not a comprehensive list but describes the most common operations done.
Neurosurgery
Dr. Zachary T. Levine is a board certified neurosurgeon with extensive experience in functional neurosurgery. He has taught DBS courses and proctored other surgeons throughout the U.S. & Europe He has served on the Board of Governors for Parkinson’s Foundation of the National Capital Area and is seen as expert in frameless DBS by the industry. He patented a technique of transplanting cells into the brain for neurodegenerative disorders.

Lesions
Surgery for brain tumors is often done for resection or removal of accessible lesions. Modern techniques such as image-guided surgery, contact lasers and endoscopes have made the treatment of these tumors easier on the patient. Many types of brain tumors can be addressed surgically, including meningiomas, schwannomas, gliomas, metastatic tumors, pituitary adenomas, and others. Our surgeons have published on the surgical treatment of meningiomas and has taught courses on resection of these tumors at the cranial base.
