Information for New Patients - How to make your first visit a success: Plan to arrive a few minutes early to complete the necessary registration and insurance information. Bring a valid Picture ID like Driver's License. Please arrange to have your medical records and any diagnostic images (MRI, CT Scan, XRays) sent in advance or bring a copy with you. The physicians like to see the actual scan, not just the report. These records may be pertinent to our evaluation of your neurological system. We will be able to better serve you with the most information available in hand. Patients should contact their primary care physician or referring physician to have records faxed to the office at 972-306-6500. You may need to sign a release to have the records transferred. The physicians like to see the actual scan, not just the report. Bring an accurate list of medications to each appointment. Be sure to include the medication name, prescribed dosage, prescribing physician's name, and how often you take each medication. We will also need information about your insurance coverage. If any of this information changes between visits, please let us know so we can update your records. If your insurance company requires a referral from the primary care physician, you must have it faxed or bring it with you. Insurance co-payment is due at time of service. We do accept Visa, MasterCard, American Express, check or cash. We accept most insurance companies but we ask that patient's check with their companies to make sure we are a participating provider. If you are covered by an insurance company that requires pre-certification and authorizations for procedures (e.g: Sleep study, EEG, EMG), please inform our office so that we can assist you. Prescription Refills: Please note that we require at least 24 hours to complete a prescription refill request. Please have your pharmacy fax your refill request to 972-306-6500 or via e-script. Neurology & Sleep Clinic in 1997. He is the medical Director of our Sleep Center. Our Sleep Lab is accredited by American Academy of Sleep Medicine (AASM) and the only one in Carrollton, TX. Dr. Shah graduated from M.P. Shah Medical School in India with honors. He completed an internship in Internal Medicine before completing a Neurology residency at S.U.N.Y in Syracuse, New York. He was selected as chief resident in neurology. Dr Shah is a Diplomat of American Academy of Sleep Medicine. Dr. Shah is a board certified in Neurology (ABPN) and Sleep Medicine (AASM and ABMS) and has over 20 years experience in patient care. He is also certified by American Association of Neuromuscular & Electrodiagnostic Medicine. He keeps his knowledge current by attending several hours of CME. In addition to diagnosing and treatment of neurological diseases, Dr. Shah offers expertise in EMG. Dr. Shah is a consulting neurologist at Texas Health Resource Plano (Presbyterian Hospital of Plano) and Baylor Medical Center at Carrollton. Memberships include Texas Neurological Society, American Society of Neuroimaging, American Academy of Sleep Medicine, Dallas County Medical Association and Texas Medical Association. He resides with his wife and two daughters. Apart from being a tennis and technology fan, he likes to exercise, read and travel. Along with his family, he organizes and serves at Health Fair to help his community.

Neurology & Sleep Clinic, we educate patients how to use and to maintain their CPAP Equipment. Even if a CPAP Equipment qualifies as a Durable Medical Equipment, it still needs to be properly maintained to function well and last long.
Carpal Tunnel Syndrome

EMG/NCS is a test used to investigate symptoms such as numbness, tingling, weakness and back/neck/limb pains. The test measures the electrical activity in muscles and nerves, and helps determine if there is damage to these structures. EMG/NCS is usually done if there is a suspicion of carpal tunnel syndrome, a pinched nerve in the neck or back, nerve damage in the feet as in neuropathy, or for muscle damage as in myopathy.
