Colonoscopy
You will be given information about the study and asked to read and sign the consent to participate when you arrive for your procedure. You will be asked to complete a short survey form at the time of the procedure and an additional survey may be mailed to you in about 4 weeks after your colonoscopy. If you have any questions about the survey, please ask your admitting nurse or physician at the center.
Irritable Bowel Syndrome

Internal Medicine
Dr. Ghazi completed her internal medicine residency at Thomas Jefferson University Hospital in Philadelphia and her gastroenterology and hepatology fellowship at University of Maryland Medical Center (UMMC) in Baltimore. After completing her fellowship, she accepted a faculty position at UMMC and Baltimore VA Medical Center in 2010. While there, she specialized in the treatment of patients with Inflammatory Bowel Disease (IBD). She was also Associate Program Director of the GI fellowship program and authored many papers and book chapters pertaining to her topic of interest. She has also presented her research at national meetings across the country.
Gastroenterology

Endoscopy
If you are a Legal Guardian for patient that is having a procedure at the Concord Endoscopy Center, please print and complete the following forms that coincide with the type of procedure that is being performed. Also, a copy of the Legal Guardianship paperwork is required.

Ultrasound
Dr. Oosterveen is affiliated with the American College of Gastroenterology, the American Society for Gastrointestinal Endoscopy, and the American Gastroenterology Association. He is board certified in gastroenterology. He has special interests in ERCP, endoscopic ultrasound, pancreatic disease and general gastroenterology.
