Golden Horizons Elder Care Services, Inc., founded in 1996 by Lisa Gradzewicz, is an in-home care agency that provides quality care to the elder population along the Connecticut Shoreline and into the Greater Hartford area. We serve four counties: Hartford County, Middlesex county, New Haven County, New London County. “Dad needs care at home…” “Mom doesn’t have anyone in the house…” “I live out of state. How can I help Mom and Dad from a distance?” We at Golden Horizons ElderCare Services, Inc., offer caring solutions to these common obstacles and more… Mission The mission of Golden Horizons is to provide home health care services to the elder population along the Connecticut Shoreline and in the Greater Hartford area, that is recognized by our clients for its excellence in its ability to personalize the care to the needs of the person being served. In this effort it is our intention to operate in a manner professionally sensitive to the concerns of the family, involve the skills and knowledge of a thoroughly trained and screened group of caregivers, and fully utilize our resources to provide the peace of mind and satisfaction that is derived from improving the quality of life for the elder segment of our population. It is our goal to allow the elderly to remain independent in the comfort of their home. Through our many services we encourage self-sufficiency and provide personal care, homemaking, and companionship. A thorough assessment is completed on each client in order to provide an individualized, comprehensive home care plan that can be monitored by our staff, including an RN review and follow-up. We are registered with the State of CT as a Homemaker-Companion Agency (Reg#HCA.0000133) and are approved by a number of Long Term Care insurers. The well-being of the elderly is our primary purpose. Amenities

Prior to the start of home care services, we conduct a complete assessment of individual needs. We then create a coordinated plan of care that addresses the needs identified in the assessment. We consult with the client, family members, and medical professionals (when appropriate) to achieve agreement and finalization on the most comprehensive plan of care to meet the needs specified. Adjustments to the plan are made by close monitoring and communication with all parties involved. There is a 4 hour minimum per shift for our hourly services. Meal preparation, wash dishes Change bed linens, do laundry Vacuum and dust/light housekeeping Kitchen and bathroom maintenance Transport and accompany to appointments, errands, Senior Center, etc. Grocery shopping Companionship: go for walks, take car rides, play cards, board games etc. A personal care attendant provides help with personal care such as bathing and dressing. They perform all the duties of a Homemaker/Companion as well as assistance with activities of daily living (ADLs): Assist with bathing Assist with toileting Assist with hair care Assist with skin and foot care Assist with medication reminders Â
