Primary Care
A Hospitalist is a provider who specializes in caring for patients who are admitted to the hospital. Our Hospitalists will manage your care during your hospitalization, coordinate and consult with specialists, evaulate your progress, and communicate your care plan with you, your family, and your primary care provider if you would like. Having our Hospitalists care for you during your stay can result in a more timely and efficient hospital experience. When you leave the hospital, your primary care provider will resume responsibility for your care.
Smoking Cessation
Since smoking and the use of tobacco products are major contributing factors to many health problems, Community Memorial Hospital has instituted a smoke-free policy that prohibits smoking on any CMH property, interior or exterior, owned or leased. This policy is for the health and well-being of our patients, visitors, and team members. Our physicians and staff are ready to assist you with smoking cessation. Please consult your physician about nicotine-replacement products or other therapies.

Emergency Care

The Passport Program offers various primary care services to employees for a low monthly fee which allows them visits to any of CMH’s family providers, along with other benefits such as Health Fit Fitness Center membership, annual flu shots, some medications and lab tests included with primary care visits, plus other benefits.
Electrocardiogram
The Respiratory Therapy staff delivers individualized diagnostic and therapeutic respiratory care procedure for adult and pediatric patients including: oxygen therapy, aerosolized medication therapy, chest physical therapy, incentive spirometry, mechanical ventilation, CPR, airway care, pulmonary function testing, breath alcohol testing, pulmonary and cardiac rehab, arterial blood gas testing, treadmill stress testing, echocardiogram testing, EEG testing, OB care, electrocardiogram and holter monitor testing.

I just wanted to thank each of you for the great job you did for our health screening again. Our guys give me compliments all the time after we do these health screenings. The Nursing department at CMH is comprised of RNs and LPNs who are committed to providing individualized quality care while integrating the family and significant others in obtaining wellness and maintaining health. Our team of nurses provides care on the patient floor, in the emergency room, in the obstetrics unit, in surgery, and in physician offices.

Joint Commission accreditation can be earned by many types of health care organizations, including hospitals, doctor’s offices, nursing homes, office-based surgery centers, behavioral health treatment facilities, and providers of home care services.
Radiology staff provides diagnostic services including MRI, CT scan, Ultrasound, Mammography, Dexascan (Bone Densitometry), Xray, and all routine diagnostic radiology procedures. Staff is also charged with performance improvement monitoring and evaluation, quality control, and radiation protection for both patients and staff.
MRI
Our current 25-bed critical access facility serves over 17,500 residents in Northwest Ohio and Northeast Indiana. Community Memorial Hospital provides a wide range of medical and specialty services including 24-hour emergency care, Musculoskeletal Institute, Specialty Clinics, the regions’ only High-Field Open MRI, and comprehensive rehabilitation services.
Radiology
Patients in a clinic or services setting must follow the following guidelines: One visitor with pediatric, elderly or special needs patients. Radiology may have one visitor for OB ultrasounds.
General surgery is a surgical specialty that focuses primarily on the torso region and abdominal organs including esophagus, stomach, small bowel, colon, liver, pancreas, gallbladder and bile ducts, and often the thyroid gland. It also deals with diseases involving the skin, breast, soft tissue and hernias.
Our general surgeon, Jeffrey Justice, MD, is one of the region's top general surgeons. In 2012, a Fort Wayne Monthly survey of 2,500 physicians named him a Top Doc in the categories of General Surgery and Breast Surgery.
Community Memorial Hospital supports our communities by offering a variety of free programming and services such as patient transportation, educational events, school tours, school programs, summer lunch programming, sports physicals, and athletic training services. Occupational Therapy
The Health Fit department provides physical, speech, and occupational therapy and cardiac rehabilitation for both inpatients and outpatients. This department also houses a community fitness center that offers exercise equipment, free weights, and fitness classes open to the public.
Aquatic Therapy
The first Mark, Milford, Hicksville joint township hospital opened in 1953, and since, Community Memorial Hospital (CMH) has been dedicated to improving the health of residents in our service area. With support from the community, we have continued to expand and improve our facility with major additions/renovations in 1958, 1982, 1985, 1988, 1993, and 1999. These expansions included physician offices, radiology services, surgical suites, and the Health Fit Rehabilitation and Wellness Center. Our most recent major addition was the construction of a new hospital facility in 2006 which included a new inpatient wing, emergency department, obstetrics department, surgery suites, café, and meeting rooms. In 2011, we constructed and opened an Aquatic Therapy Pool. In 2016, we completed a reinivation of lab and respiratory services. In 2019, an Orthopedic specialist clinic was developed within the hospital.
The specialty physicians at the Musculoskeletal Institute of Community Memorial Hospital offer a wide range of surgical and non-surgical treatments for muscle, nerve, and joint pain. The Institute currently includes the following specialties: orthopedics, physical medicine and rehabilitation (physiatry), podiatry, chiropractic, and neurosurgery. Clinic locations include Hicksville and Ft. Wayne (IN).
