Pain in the ball of the foot not associated with calluses can be a result of inflammation of a tendon into the toe, inflammation of the joint, or due to a pinched nerve called a Neuroma. A neuroma will also often cause a burning pain into one or more toes. Another cause of pain in the ball of the foot is arthritis. Arthritis will usually affect multiple joints in the ball of the foot. Common types of arthritis are rheumatoid arthritis, osteoarthritis and degenerative arthritis. Degenerative arthritis most commonly affects the big toe joint. The big toe joint is often stiff causing jamming (Hallux Limitus). Over time the jamming of the joint will cause an enlargement on the top of the joint.

A doctor will evaluate the condition and range of motion of the tendon. Other imaging devices may be used to evaluate the condition of the tendon such as ultrasound imaging and x-rays.
MRI
One of the most commonly asked questions is Will the screws set off metal detectors ? The answer is no. These screws are non-magnetic and will not set off metal detectors. Additionally, you with not pick up radio waves. These statements are myths and have no relevance. In having these types of screws in your foot, you can also have a MRI test with complete safety.

Bunion Surgery
The cost of bunion surgery can vary greatly depending upon the severity of the bunion, the procedure, insurance coverage and facility charges. However, the Washington Bunion Center, a division of Issaquah Foot and Ankle Specialists, offers bunion surgery at our own ambulatory surgery facility located within our offices. Because we can perform the majority of our bunion surgeries in our own facility, in a comfortable outpatient setting, patients typically spend much less on their procedure and enjoy a more comfortable environment.
Metatarsal Surgery
Calluses on the bottom of the foot can be painful. These calluses are caused by an abnormal alignment of the bones behind the toes called metatarsal bones. When there is an abnormal alignment of one or more of these bones, they will experience excessive weight and pressure. The skin thickens in response to this pressure forming the callus. Treatment consists of periodic trimming or shaving the callus, padding the shoes to remove the pressure, functional shoe orthotics, or surgery. Surgery should not be contemplated until the use of orthotics has been determined to have failed. The surgery consists of fracturing the involved metatarsal bone to realign it (See metatarsal surgery). Following the surgery, the patient should wear an orthotic to prevent the occurrence of new areas of callus.

Gangrene
Additional abnormalities encountered routinely with ABS include webbing of the fingers or toes, progressive lymphedema (swelling), clubfoot, stunted growth of the small bones in the fingers and toes and limb length discrepancy. Less common findings include: pseudarthrosis, metatarsus adductus, peripheral nerve palsy, dystrophic nails, postnatal gangrene, cleft lip and palate, skin-tube pedicles, dislocated hip visceral body wall malformations and eccentric craniofacial synostosis defects. Owing to the fact that no two cases are exactly alike, only some of these above-mentioned defects are present in each individual case. Other congenital brain abnormalities, cardiac malformations, short statue, spina bifida, and added miscellaneous conditions reported in the literature probably represent coincidental findings. Fetal death associated with amniotic band strangulation of the umbilical cord has also been reported.
Ingrown Toenails
If have ever experienced an ingrown toenail you already know that early intervention is important. The level of pain, discomfort and potential for infection will increase when ingrown toenails are left untreated. Infection is more likely when the nail causes a break in the skin. This open wound allows bacteria to enter the wound and often our feet are exposed to more bacteria than other parts of our bodies. Also, our feet often spend long periods in the warm moist environments that footwear creates. This environment often contributes to discomfort and potential for infection.

Ulcer
Callus build up on the foot is due to abnormal pressure and friction as you stand and walk. It is important that the callus not get to thick or the skin under the callus can break down and cause an ulceration. It is not uncommon for me, when treating a diabetic patient with calluses on the feet, to trim a callus and find an ulcer under the callus. If the callus is not trimmed, then the infection can progress into the bone or deep into the foot. Good nutrition and vitamin supplements will help with healing but the most important issue is adequate blood flow. If you have bad circulation ask your doctor about hyperbaric oxygen treatment.
Calluses
The podiatric physician cares for people of all ages. Common disorders of feet include bunions, heel pain/spurs, hammertoes, neuromas, ingrown toenails, warts, corns, calluses, sprains, fractures, infections, and injuries. If your podiatric surgeon is certified by the American Board of Podiatric Surgery, he or she has successfully completed a credentialing and examination process and has demonstrated knowledge of podiatric surgery. This includes the diagnosis of general medical problems and surgical management of foot diseases, deformities, and trauma of the foot, ankle and related structures.
Fungus
The most common cause of yellowed, thick and /or deformed toenails is a fungal infection of the toenail. The fungus that infects the nail, most commonly, is the same fungus that causes athletes foot. It tends to be slowly progressive, damaging the nail to a greater and greater degree over time. The infection usually starts at the tip of the nail and works its way back. It usually is not painful and often not noticed until it has gotten well established. A single toenail or any number of nails can be affected. It can also occur on just one foot. Over time, the nail becomes thickened, crumbly, and distorted in appearance. Sweaty feet contribute to the initial infection process and contribute to its spread. The fungus prefers an environment that is moist, dark, and warm, which is why it affects the toenails much more often than fingernails. It does not spread through the blood stream. The infection limits itself to the nails and skin. It is often found in association with areas of dry scaly skin on the bottom of the foot or between the toes. The dry scaling skin is frequently found to be chronic athletes' foot. It is not highly contagious, and family members are almost as likely to contract it from some other source as they are from the family member who has the infection. Keeping common showering areas clean is recommended, and sharing shoes should be avoided.
Lesions
If you suspect you have plantar warts, you may have the following symptoms: pain or tenderness while walking, a lesion that interrupts the ridges in the skin of your foot, small fleshy lesions on the bottom of the foot, or a callus where a wart has grown inward over a well-defined spot on the skin.
Psoriasis
Not all thicken or yellowed toenails are caused by a fungal infection. Injury to a toenail can cause the toenail to grow in a thickened or malformed fashion. This can be due to an established fungal infection or may be due to the damage caused to the nail root when it was injured. In these instances, treatment with anti-fungal medications will not correct the malformed nail. Other causes of thickened toenails are small bone spurs that can form under the toenail and psoriasis. Taking a scraping of the toenail and culturing it makes the diagnosis.

Sports Medicine
Sports Injuries. Athletes can suffer a number of foot and ankle injuries. Inflammation in the joints can cause turf toe, ankle sprains, sesamoiditis, and even tendon dislocation, while trauma and repetitive stress can cause fractures in the toes, ankle, and midfoot. Most conditions involve immobility in a boot or shoe to relieve stress while the bone is healing, but may involve surgery if the trauma is extensive. We always offer an aggressive rehabilitation program for athletic foot injuries, including maintaining ankle stability, preventing further injury, and returning the athlete to pre-injury functional level.
Physical Therapy
Treatment depends upon the specific cause of neuropathy. If the neuropathy was caused by vitamin deficiency, diabetes, infection, or toxic substances, treating the condition can lead to the nerve healing and sensation returning to the area. If the nerve has died, however, sensation may never return to the area. Pain medication may be prescribed for less serious symptoms. Certain topical creams may work to bring back sensation. Electrical nerve stimulation may be used for a period of time to stimulate nerves, and physical therapy can strengthen muscles and improve movement. Surgery may be necessary if pressure on the nerve is causing the neuropathy.

Ankle fractures are common injuries that are most often caused by the ankle rolling inward or outward. Many people mistake an ankle fracture for an ankle sprain, but they are quite different and therefore require an accurate and early diagnosis. They sometimes occur simultaneously.

Achilles Tendonitis
Achilles tendonitis tends to occur more frequently in older athletes than in younger athletes. As a person ages into their thirties and especially into their forties and fifties, the ligaments and tendons of the body tend to lose some of their stretchiness and are not as strong as before. This predisposes older individuals who are active in running and jumping activities, to tendon injuries such as Achilles tendonitis. However, Achilles tendonitis can also occur in teenagers who are very active in running and jumping sports.

Bunions Problems
St metatarsal. It involved cutting the bone and realigning the joint. It is usually secured with 1 or 2 screws. Most patients can walk on this in a boot following surgery. The average time to heal is typically 4-8 weeks. This procedure is primarily reserved for smaller bunions. It is the most popular bunion procedure done to date.
Calluses
Fungus Toenails
Toenail fungus, or onychomycosis, is an infection underneath the surface of the nail caused by fungi. When the tiny organisms take hold, the nail often becomes darker in color and smells foul. Debris may collect beneath the nail plate, white marks frequently appear on the nail plate, and the infection is capable of spreading to other toenails, the skin, or even the fingernails. If ignored, the infection can spread and possibly impair your ability to work or even walk. The resulting thicker nails are difficult to trim and make walking painful when wearing shoes. Onychomycosis can also be accompanied by a secondary bacterial or yeast infection in or about the nail plate.

Surgery. We try to relieve most conditions without surgical intervention; however, our podiatrists are board-certified in foot surgery. If it becomes necessary to correct a bunion, hammertoe, or ingrown nail surgically, our surgery center is connected to our offices, ensuring easy access to relief of foot and ankle pain. Over ninety percent of the surgeries we perform are done in our next-door surgery suite.
Heel Spurs

Ingrown Toenails
A toe, which is red, swollen and perhaps warm to the touch are signs that your toenail is ingrown. If you notice that your nail appears to curve downward into the skin this is a sign that your ingrown toenail can lead to infection in your toe.

Early intervention is important. Surgery is often performed to address this condition. However, there are alternatives to surgery for Mortonâ€™s neuroma. In fact a number of our patient's recently received dehydrated alcohol injections and are doing exceptionally well. Statistically the success rate has even been slightly higher with these types of injections opposed to surgery. We use high definition ultrasound imaging to guide the injection directly to the painful area. We can also incorporate nerve stimulation to significantly reduce any sensation associated with the injection. Research has indicated that success rates from surgical treatments are between 80-85%. The latest research of dehydrated alcohol injection therapy for Mortonâ€™s neuroma has been 89%. Therefore we believe we are well on our way to ending the pain associated with Mortonâ€™s neuroma with less invasive treatments. Plantar Fasciitis
Plantar Fasciitis (pronounced plan-tar fash-ee-eye-tis) is a common cause of heel and foot pain in adults. â€œPlantarâ€ means the bottom of the foot; â€œfasciaâ€ is a type of connective tissue, and â€œitisâ€ means â€œinflammationâ€ .
Podiatric Deformities
Family history seldom reveals any direct inheritance pattern, since the syndrome occurs in no particular association with know genetic or chromosomal disorders. Karotypes are virtually always normal, and the syndrome is almost always sporadic in nature. Infants of young, black women who have been pregnant more than once (<20 years, more than one pregnancy) show the highest prevalence. incidences of malformations seen in the hand are two times as common as accompanied foot deformities.
