Mohs Surgery

How we look plays a big role in the lives of most people. For some, the reflection we see can cause displeasure due to accidents, birth defects or other signs such as premature aging. Many of these conditions can be improved with surgical and nonsurgical procedures and many people choose to put their faces in the hands of a facial plastic surgeon. A facial plastic surgeon specializes in performing cosmetic and reconstructive surgery to the face, head and neck region. Concentrating on the complex anatomy, physiology, pathology, bio-chemistry and bacteriology of all the structures of the head and neck, provides Dr. Heinrich a strong focus on the anatomical area being treated.

Plastic Surgeons

Botox
Dermal fillers and injectables can restore youthful fullness to your face, plump the lips, enhance shallow contours or soften facial creases and wrinkles. Dermal fillers in conjunction with an injectable, such as Botox, can offer many of these benefits without downtime. At Pacific Coast Cosmetic & Laser Medical Center, Inc., Dr. Heinrich and his team will assess your needs and recommend one or a combination of treatments to achieve your desired results.
Brow Lift
A forehead lift will remove or reduce horizontal creases across the forehead, furrows between the eyebrows and sagging eyebrows. Due to the aging process, genetics, stress or muscle activity, facial muscles and tissues can loose elasticity causing wrinkles and furrowed lines. Forehead lifts are commonly performed on people in their 40s to 60s, however they are also done on younger individuals who have sagging, droopy eyebrows often resulting in a sad facial expression. Not infrequently, a brow lift is combined with Blepharoplasty (an eyelid tuck) or face lift to provide a harmonious rejuvenation. Dr. Heinrich will meet with you for a consulation and determine if you are a good candidate for a forehead lift. He will assess your health and medical history, discuss with you the forehead lift process in detail and answer all of your questions.
Eyelid Surgery
In upper eyelid surgery, Dr. Heinrich will first mark the individual lines and creases of the lids in order to keep the scars as invisible as possible along these natural folds. The incision is made, and excess fat is removed or re-positioned, and then the loose muscle and skin are removed. Fine sutures are used to close the incisions, thereby minimizing the visibility of any scar.
Liposuction
Because the skin must mold itself to new contours following surgery, candidates for liposuction are generally physically active, near their ideal weight and have good skin tone and elasticity. Additionally they must have realistic expectations and be psychologically stable.
Rhinoplasty
There are two fundamental approaches to rhinoplasty – open and closed. Open rhinoplasty involves a small incision to the columella (the fleshy end of the cartilage between the nasal passages), whereas the closed rhinoplasty is performed without cutting the columella. Large nostrils may be reduced through tiny incisions at the outer base of the nostrils. Dr. Heinrich will outline the differences between closed and open rhinoplasty and plan what is best for you. Depending on the complexity of the surgery, rhinoplasty can be done either with local or general anesthesia.
Plastic Surgery
Fat injection, or fat grafting, has broad applications in plastic surgery. It can be used to create fuller or more defined lips, fill minor creases between the base of the nose and the corners of the mouth (nasolabial folds), correct facial asymmetry or enhance cheeks for a more prominent contour. It produces safe, long-lasting, and naturalappearing results as volume is replaced to tissues where fat is diminished.
Face Lift
The trend in facelifts is to have surgery at a younger age. The typical patient is likely to be in their 40s or 50s and may be just beginning to see some signs of aging, however the surgery can be performed as late as your 70s or even 80s. When performed at a younger age the results last longer because there is more elasticity in the tissue. Younger patients may also benefit from a simpler procedure such as a mini-facelift, with faster healing and a shorter incision. Timing for facelift surgery ultimately depends on when the patient is mentally and aesthetically ready. It is important not to wait too long for the procedure because the skin becomes less elastic and deeper lines form, it becomes harder to achieve a truly natural looking result.
Neck Lift
If you have jowls, a double chin, a turkey wattle or any other cosmetic concerns about the appearance of your neck this procedure may be appropriate for you. Whether your neck is affected by age, your genes, the pull of gravity or the harsh effects of the sun and wind, neck lift surgery (platysmaplasty) or other neck contouring and rejuvenation procedures may be options. A neck lift removes excess sagging skin under your jawline and on your neck. For those with bands or cords, the neck lift may involve adjusting and tightening the platysma muscle, if it has begun to separate and sag. If needed, the neck lift can include the removal or contouring of excess fatty tissue, some of which may be located below your platysma muscle.
Blepharoplasty
A certain degree of swelling and bruising is normal. Cold compresses, as well as head elevation when lying down, will enhance healing and relieve discomfort. Dr. Heinrich will also prescribe medication for discomfort. For a week and a half following blepharoplasty, you will clean the eye area (the eyes may feel sticky, dry, and itchy). Eyedrops may be recommended. A list of activities and environments to avoid in the weeks immediately following surgery will be provided. Permanent stitches will be removed in three to five days after surgery. Self-absorbing stitches will dissolve on their own.
Nose Surgery
Chin augmentation using surgical implants can alter the underlying structure of the face, providing better balance to the facial features. This can take the form of chin height reduction or chin rounding by osteotomy, or chin augmentation using implants. Rhinoplasty (surgery of the nose) specialists often suggest that changes in chin size or shape may enhance a profile as much as rhinoplasty. It is common to recommend chin surgery in addition to nose surgery to achieve facial balance and harmony. Fortunately, this is a relatively straightforward procedure that can make a major difference.
Otoplasty

Ear surgery creates a natural shape, while bringing balance and proportion to the ears and face. Correction of even minor deformities can have profound benefits to appearance and self esteem. If protruding or disfigured ears bother you or your child, you may consider this procedure. Specifically ear surgery can treat: Overly large ears — a rare condition called macrotia, protruding ears occurring on one or both sides in varying degrees — not associated with hearing loss, and adult dissatisfaction with previous ear surgery.
Scar Revision
Scar revision techniques and procedures provide a selection of options to help reduce the appearance of various types of scars. The best method for treating your scar depends on the location, the unique way your body heals, and the specific characteristics of the existing scar tissue. Scar revision surgery involves removing the old scar and closing the wound using advanced techniques, such as complex flap closure or layered closure. The procedure is done on an outpatient basis under local anesthesia. Depending on the characteristics of the scar, pharmaceutical tissue substitutes may be used to replace missing skin, or tissue expansion techniques can be used to stretch nearby areas of healthy skin, in order to provide ample tissue for optimal wound closure. At your consultation with Dr. Heinrich he will help determine which procedure can best meet your individual needs. As with all types of cosmetic procedures, it is important to have realistic expectations and a complete understanding of the process.
Laser Resurfacing
Non-wounding (nonablative) laser resurfacing: (IPL™) Heat energy from the laser damages the collagen beneath the skin and stimulates the growth of new collagen, which tightens underlying skin and improves skin tone and appearance. This approach requires a series of treatments. A topical skin-numbing agent may be used for pain control, or none may be needed. The skin may be temporarily red or swollen. Patients may be able to resume normal activities right away.
Dermabrasion
Dermabrasion: A small motorized device with an abrasive wheel or brush is used to remove the outer layers of skin. When the skin grows back, it’s usually smoother and younger looking. This procedure may require numbing the skin. Recovery time depends on the extent of the procedure.
Facial Implants
Individuals who would like to add contour and definition to facial features can benefit from facial implants. Cheek implants can add volume to the cheeks for a more youthful and defined appearance. Cheek implants are most often used to restore sagging mid-facial soft tissues, thus postponing the need for a facelift. However, this procedure can also be performed in conjunction with a facelift to better define the enhancement. There are many options for aesthetic cheek surgery, depending on your particular appearance, and the look you wish to attain. The procedure adds volume to the cheek by effectively augmenting the bone structure of the cheek region, enhancing the foundation of your mid-face.
Chin Augmentation
Cheek implants can be made of solid silicone or other non-reactive materials such as Medpor. During your consultation with Dr. Heinrich he will discuss with you the best approach and preference to meet your aesthetic goal. A small incision is made under the upper lip, in the lower eyelid, or in the hairline and an implant with a biologically correct shape is inserted over the natural cheekbone contour. The implant is secured either by designing a well fitting pocket, or affixing it to soft tissue or bone. The procedure is commonly combined with other procedures, such as a face lift or chin augmentation. Cheek implant surgery is performed under local anesthesia and light sedation. The procedure is performed on an outpatient basis and lasts approximately one hour.
Forehead Lift
Based on variations in how men and women age and on new advances in medical technology, different methods are used to perform this procedure. A forehead lift typically takes between one to two hours to perform. It is most commonly performed under IV sedation or twilight anesthesia. The forehead lift surgery is done one of two ways. The conventional method is through an incision behind the hairline. The newer forehead lift technique is done using an endoscope and requires minimal incisions. If the conventional method is used, an incision is made at the hairline from the ears across the top of the forehead. The forehead skin is pulled back and any excess skin, muscle and tissue is removed. The incision is stitched closed and wrapped with sterile bandages.

Treatment for skin cancer and the precancerous skin lesions known as actinic keratoses varies, depending on the size, type, depth and location of the lesions. Small skin cancers limited to the surface of the skin may not require treatment beyond an initial skin biopsy that removes the entire growth.
Chemical Peels
