I am a physician who has committed his practice to the medical treatment of the overweight and obese patient for the past ten years. As you know obesity is a genetic, metabolic, chronic and progressive disease much like its “co-morbid cousins” diabetes and hypertension. Chronic diseases require long term treatment for control not cure. The traditional paradigm for obesity treatment; ie, diet, exercise and behavioral modification has proven ineffective for the vast majority of patients (95% failure rate at 3-4 years) and thus the need for a more aggressive medical approach. The use of combination drug therapy to suppress appetite, control cravings, improve insulin sensitivity and lower the metabolic set-point has proven highly successful for “long-term” weight control in most of my patients. Since September 1997 I have used a combination of phentermine and an SSRI such as prozac, zoloft, celexa or luvox as initial treatment in probably 4000 plus patients. Paxil the other SSRI is avoided due to its weight gain potential. Since many overweight/obese patients are symptomatic for mild depression, carbohydrate cravings, migraine, irritable bowel, fibromyalgia, premenstrual symptoms; the use of an appropriate SSRI is highly effective for treatment of these “low serotonin” symptoms. The additional use of wellbutrin, effexor, serzone, glucophage or xenical is helpful in selected and resistant patients. I have found phentermine alone or meridia to be useless for most patients for long term success. Although some patients may do better with one SSRI vs another, or possibly using serzone or effexor in place of an SSRI; I rarely have to discontinue the medications due to side effects. The use of safe and effective medications in addition to diet and exercise protocols I believe is key for long term weight loss maintainence.
Medical Weight Loss
Metformin (GLUCOPHAGE) significantly increases weight loss when combined with reduced-calorie diet compared to a placebo concludes a new study. The average body mass index was reduced from 31.3 to 28.0 after three months in patients given 850 mg of metformin daily in addition to a diet compared to 30.3 in those given a placebo. This reduction with metformin Is equivalent to roughly 19 pounds in women and 23 pounds in men compared to 6 to 7 pounds for those given a placebo. The study involved 40 male and 60 female obese type II diabetics with an average age of 54 years who were put on a diet of 1,500 calories per day for men and 1200 calories for women.
Nutritional Counseling

Originally from Philadelphia, Dr. Skversky received his medical degree from Hahnemann University (formerly Hahnemann Medical College) in 1970. He completed his family practice residency at Hoag Memorial Hospital in Newport Beach, CA, in 1973 and became a diplomate of the American Board of Family Practice the same year. Dr. Skversky is a staff physician at Hoag Memorial Hospital, a member of the American Medical Association, Orange County Medical Association, and the American Society of Bariatric Physicians. Dr. Skversky had been a family practice physician in Newport Beach for more than 22 years before practicing bariatrics full time.
Emergency Care
A recent editorial from the New England Journal of Medicine (December 1996) estimated about 28 cases/million person years of exposure (1 in 40,000 patients). This is about equal to the risk of a woman on birth control pills having a fatal pulmonary embolus (blood clot in the lungs) or a patient receiving penicillin and dying from an acute anaphylactic reaction. In twenty-five years as a family practice physican, including eight years as an emergency room physician, I never had a patient with either of these unfortunate medical outcomes. The recent death of a woman in Massachusetts (February 1997), if determined to be associated with the use of Phen/Fen or Redux, would be the first documented case in this country in the past fourteen months.

Obesity has been shown to be a significant risk for cardiovascular disease, hypertension, diabetes, and dysilipidemia (elevated cholesterol and triglycerides). In addition, there often are cosmetic and psychological issues that overweight people must learn to cope with. I believe that weight loss (in an attempt to get as close to ideal body weight as possible) is as important for general health and longevity as quitting smoking is to the prevention of lung cancer. Recently, Time magazine did a cover story on “The Girth of a Nation.” One of the ways to change that story to the “Health of a Nation” would be to inform the more than 50 million overweight people in our country of the availability of pharmacotherapy as an important adjunct for long-term weightloss.
