Pediatric Care
Port Orange Pediatrics is the office of Ruben Lopez, M.D., who is kind enough to allow Drs. Doug Stein and John Curington to use his office periodically (on certain Friday afternoons) to provide vasectomy services. For exact dates, see our Outfield Schedule. The fee is $590, less for men with most health insurance plans. A deposit of $100 is required to be scheduled (see below). The counseling process can be done online by ( 1 ) reviewing our vasectomy page, ( 2 ) viewing the online counseling video, and ( 3 ) reading the vasectomy info sheet. So only one visit is required, but you will have the office, home, and cell phone numbers of Drs. Stein and Curington so that you can always reach them if you have questions after your procedure. You will also be given a mailer, so that you can send a semen sample to our Tampa office 1 2 weeks after your vasectomy to be sure that your semen is sperm-free.
Primary Care
Most vasectomy candidates know someone who has had a vasectomy. Indeed, many are referred by men who have had vasectomies here. Others are referred by their primary care doctors or their wives' gynecologists who in turn have received feedback from other patients. These are up-front referrals and testimonials in themselves.

Emergency Care
On Wednesday March 20, Thursday March 21, and Friday March 22, 2024 at his North Tampa / Lutz office, Dr. Stein will host training days for Dr. Amit Grover, a Family Medicine and Urgent Care physician in Washington. He would like to join a busy vasectomy practice and needs training. Because of the reduced rate and the need for men who are not likely to postpone to a later date at the last minute (trainees cannot learn from men who don't show up), we ask for the full (discounted) payment at the time Mar 20-22 training cases are scheduled, refundable until 14 calendar days prior to the appointment date, non-refundable thereafter. Treatment: In certain cases, a woman may use a pessary, a device to block the urethra or to strengthen the pelvic muscles. In addition, medications also can be helpful in controlling muscle spasms in the bladder or in strengthening the muscles of the urethra. Some drugs can help to relax an overactive bladder. Kegel exercises are another method that can be used to help control urinary incontinence. These exercises help tighten and strengthen the muscles in the pelvic floor. Strengthening the pelvic floor muscles can improve the function of the urethra and rectal sphincter.

Gynecology
* - Since reversal attempts often do not lead to pregnancy, vasectomy should be considered an irreversible form of contraception. Statistics show that men who undergo vasectomy in their 20's are more likely to seek vasectomy reversal than men who have vasectomies later in life. In fact, men in their 20's and with fewer than 2 children should read our Special Message for young men. Before choosing vasectomy, couples, especially couples in their 20's with fewer than 3 children, should consider all other forms of reversible contraception including birth control pills, shots, patches, and implants; the IUD (intrauterine device); and barrier methods such as the condom and diaphragm. A nice review of these options is in the Planned Parenthood website, and HERE in VasWeb. Couples using barrier methods should also be aware that, for those occasions when they are just "careless", EC ( emergency contraception, or the "morning after pill") is readily available at most women's health services sites such as private primary care and gynecology offices and Planned Parenthood clinics. EC is effective at preventing pregnancy for up to 72 hours (perhaps longer) after the "careless" encounter (visit www.not-2-late.com for a list of EC providers in your area). Men should not proceed with vasectomy if they feel they are being pressured to do so, if their private lives are temporarily unstable due to marital discord, or if they have not considered all reversible alternatives. For many stable men, circumstances change, and all men, especially younger men, should consider sperm storage as insurance against regret over vasectomy in the event of a major life change.

Pregnancy begins with a fertilized egg. Normally, the fertilized egg attaches itself to the lining of the uterus. With an ectopic pregnancy, the fertilized egg implants somewhere outside the uterus. An ectopic pregnancy typically occurs in one of the tubes that carry eggs from the ovaries to the uterus (fallopian tubes). This type of ectopic pregnancy is known as a tubal pregnancy. In some cases; however, an ectopic pregnancy occurs in the abdominal cavity, ovary, or neck of the uterus (cervix).
Labor and Delivery
2 to 10 out of 100 IUDs are pushed out (expelled) from the uterus into the vagina during the first year. This usually happens during the first few months of use. Expulsion is more likely when the IUD is inserted right after childbirth or in a woman who has not carried a pregnancy. When an IUD is expelled, a woman is no longer protected against pregnancy.

Kidney Stones
Certainly not. The vas tubes are most easily and safely divided under direct vision with a fine surgical scissors. But the expression "LASER" has great popular appeal, and use of laser energy in the performance of a simple vasectomy serves no purpose but to play up to this popular appeal. Lasers have proven indispensable for certain types of retinal (eye) and skin procedures, and they offer an alternative, though not necessarily better, means for destroying tissue (prostate and certain tumors) and kidney stones. But a laser (like any other form of light) cannot pass through opaque tissue without burning a hole in it, so a laser cannot be magically directed at internal organs such as the vas tubes without an access opening in the same way that sound waves can be used to destroy kidney stones without an incision. Prostate Cancer
The February 17, 1993 issue of the Journal Of The American Medical Association contained 2 studies (by the same research group) that suggest that vasectomy was associated with a small increased risk of prostate cancer in their study groups (almost 30,000 patients in 1 study and almost 40,000 patients in the other study). Because the question was initially raised by 2 studies back in 1990, the World Health Organization convened a 1991 meeting of 23 international experts to review all research regarding vasectomy and prostate cancer. They concluded that there was no plausible biologic mechanism for a relationship between vasectomy and prostate cancer. Some medical researchers interpreted the small increased risk noted in the 1993 studies as a weak association that may be due to chance or bias. Neurolysis
Congestion, tender buildup of sperm and white blood cells upstream from or at the vasectomy site, can occur anytime after vasectomy, but usually goes away with use of an anti-inflammatory drug such as aspirin or ibuprofen. About one in 2000 patients will experience chronic post-vasectomy discomfort (PVPS or Post-Vasectomy Pain Syndrome) severe enough that he will seek vasectomy reversal or neurolysis (division of the sensory nerves coming from the testes). A larger percentage (the medical literature says 1-15%) may have milder forms of chronic pain that can affect quality of life but not severely enough to seek surgical therapy.

Effects of condition/treatment: With ultrasound, it's now much easier to determine whether the embryo has died or was never formed. Either finding means that a miscarriage will definitely occur. In this situation, women may choose let the miscarriage progress naturally, pursue medical intervention with medicine to speed along the expulsion process, or opt for surgical treatment.
MRI
There have been no clip-related problems in the 38,000 or so men who have undergone vasectomy by Dr. Stein since he began using this method in 1990, and no reports of problems in the medical literature of which we are aware. Surgeons have used hemoclips for many years to occlude bleeding blood vessels during many operations in the abdomen and chest, sometimes over 50 clips in a single procedure. Hemoclips have been used by vasectomists for decades. In fact, during vasectomy reversal procedures, as many as 5 clips can be discovered on each side, clips of which the patient was unaware and which could not be felt by the surgeon before the reversal. Most hemoclips are made of medically inert titanium, the same metal used for dental implants, many artificial joints, and mechanical heart valves. Not ferromagnetic, titanium will not interfere with MRI studies and the small amounts used in hemoclips and dental implants do not set off metal detector alarms.
Chemotherapy
For some men, sperm banking is almost a necessity. Men diagnosed with cancer during their early 20's face a course of chemotherapy likely to provide a cure but also likely to adversely affect sperm production. Their only hope of fathering children is with preservation and storage of sperm obtained prior to chemotherapy. For years, sperm banks have been providing this service.

Wound Care
Infection is also a rare complication. Among the first 24,000 vasectomy patients served by Dr. Stein, six infections have occurred (infection rate 1 in 4000 ): two patients had prolonged discomfort and progressive swelling on one side, not responsive to oral antibiotics, eventually maturing to a painful walnut-sized abscess requiring office drainage through a half-inch incision and a two-week period of local wound care. Four other milder infections (swelling unresponsive to anti-inflammatory medications) responded to oral antibiotics.
Joint Replacement
Hemoclip After the vas is divided, the upper (abdomenal) end is cauterized and allowed to slide back into its sheath, while the lower (testicular) end is held outside the sheath. A tiny hemoclip (the size of a grain of rice) is then used to close the empty portion of the sheath between the 2 ends. It would be like making a lengthwise opening in a wire's insulation, reaching in and dividing the wire, lifting one cut end out through the insulation, then putting a clip on the empty portion of the insulation, thereby holding one end outside and one end inside, the insulation itself serving as a barrier between the two ends. Most hemoclips are made of titanium, a non-ferromagnetic metal used for many types of implanted medical devices such as dental implants, heart valves, and joint replacement.
Vasectomy
In February, 2022, without notice to or approval from us, CIGNA lowered payments for vasectomy from $461.59 to $438.28. We quietly went along with that for a while, then, beginning May 31, 2022, we noticed that reimbursement for patients with "CIGNA Open Access" was $255.66. On July 13, we tried to contact a CIGNA representative at a phone number on the EOB (insurance payment receipt) (866-494-2111), the representative told us to call Provider Relations (PR) at 888-663-8081. The representative at PR said that there was nothing she could do to negotiate rates, and she provided email address intake_pdm@cigna.com, but no name of anyone with whom we could discuss our agreement with CIGNA. We explained the situation and concluded with "Please let this email serve as notice that future CIGNA patients will be charged $461.59 before services are rendered, regardless of the plan, then reimbursed whatever CIGNA pays us. Non-response to this email will be considered acceptance of this new policy."
Hysterectomy
Effects of condition/treatment: If the baby has not been delivered, then an emergency C-section is scheduled. If damage to the uterus is not too severe and the bleeding is easily controlled, a hysterectomy can be avoided. Doctors do their best to repair the uterus. This process generally calls for a blood transfusion for the mother, due to the amount of blood loss, plus antibiotics to prevent infection. Treatment involves removing the baby safely through the following methods: Performance of intrauterine, instrumentation (forceps), and fundal pressure. 21 inches allows me to stand upright in good posture. The magnifiers are mounted into bifocals which provide me with good distance vision and reading-glasses type magnification around the 2.5x lenses, so I keep them on through a whole vasectomy day. They are so important to me that I keep a second 2.5X pair with my travel gear (for vasectomies outside my main office) so that I never have to worry about forgetting my home office pair when I travel. Even if you perform only a few vasectomies, they are also very useful for a wide variety of office tasks, including examining skin lesions and repairing or cleaning fine instruments.
