Nutritional Counseling
Treatment Alternatives - I may use and disclose health information to tell you about or recommend possible treatment options or alternatives that may be of interest to you. For example, I may use your information to determine whether you qualify for a nutritional counseling program.
Primary Care
This will be determined by the doctor. If this is required, you will need to see your primary care physician for pre-op testing. This must be done no less than one week prior to surgery.

The Glaucoma Center, P.C. participates with the health insurance plans listed below. Our billing team is happy to assist you and answer any questions you may have with regard to your insurance plans and our financial policies.
Ophthalmology
As you know, state and federal guidelines are evolving rapidly as this situation progresses We are actively monitoring for new recommendations from trusted health authorities, such as the CDC and the American Academy of Ophthalmology, and will update our policies as necessary.
Macular Degeneration
Dr. Hu graduated from the University of Rochester with a Bachelor of Science in Microbiology. She attended optometry school at the University of California Berkeley School of Optometry. She completed her residency inocular disease at the Baltimore Veterans Affairs Medical Center. As a resident, Dr. Hu had extensive training in treating and managing ocular conditions such as glaucoma, macular degeneration, and diabetic eye disease. She also worked closely with faculty from The Johns Hopkins Wilmer Eye Institute and experts in various sub-specialties.
Laser Eye Surgery
Your physician has recommended that you undergo eye surgery. We are sure you have many questions concerning the surgical procedure and listed below are some of the most commonly asked questions and answers. If further information is needed, please do not hesitate to contact our office and we will be happy to assist you.
Cataracts
Dr. Servinsky is board certified by the American Board of Optometry. She is a therapeutic licensed optometrist who treats and manages eye disorders and diseases, prescribes medications, and provides pre and post-operative care for cataract, glaucoma, and laser surgeries.
Cataract Surgery
"Cataract" refers to a clouding of the natural lens in your eye. It is usually aged-related, but could also be due to medications, diabetes, inflammation or previous trauma. If a cataract has caused blurring or doubling of your vision, glare, difficulty driving at night or has stopped you from doing activities you enjoy, it may be time to consider cataract surgery to remove this cloudy lens.
Eye Exam
During your initial visit to The Glaucoma Center, there are a series of diagnostic tests that will be performed along with your comprehensive eye exam. Our state-of-the-art equipment uses the most advanced technology to determine whether or not you have glaucoma and if so, how severe it may be. Our doctors will review testing and eye examination results with you, and go over treatment options if needed. After your initial consultation, these tests may be performed again on a yearly basis or as deemed necessary by our doctors, to ensure that every effort is made to preserve your vision.

Mental Health
This notice advises you about the ways in which I may use and disclose health information about you. It also describes your rights to access and control your health information. 'Health Information' is information about you, including demographic information, that may identify you and that relates to your past, present or future physical or mental health or condition and related health care services. This notice also describes your rights and explains certain obligations I have regarding the use and disclosure of health information.
