Susan S. Kim, MD is a cardiac electrophysiologist and director of the Cardiac Implantable Electronic Device Clinic at Northwestern Medicine. Dr. Kim is considered an expert in the management of patients with heart rhythm disorders. Pregnancy
Prior to choosing to have a Mommy Makeover, there are a few items to consider. First, it is often recommended that a woman wait until she is finished having children. Since you are making a personal and financial investment in the surgery, it may be in your best interest to wait until you have completed your family.After you have decided to undergo a Mommy Makeover, you should wait until the body has recovered from pregnancy and childbirth. Typically, this will occur around six months following childbirth. A woman should also be finished breastfeeding so the breasts are able to return to their natural size and shape.
Labor and Delivery
Many women notice loose, sagging skin in their abdominal region following childbirth. During pregnancy, the skin is stretched quite taut, and upon giving birth, the skin loses its elasticity and cannot recover its initial shape. A tummy tuck removes any excess fat and skin from the stomach area, so that the muscles in the abdominal area are able to tighten, revealing a slimmer, smoother stomach.

Ovarian Cancer
Abnormalities within the BRCA1 (BReast CAncer gene one) and BRCA2 (BReast CAncer gene two) genes are associated with increased risk of breast cancer. BRCA 1 and 2 are “tumor suppressor” genes that, when functioning correctly, work to ensure normal breast development. Upon mutation, however, these genes no longer suppress tumor formation but in fact increase the likelihood of tumor development and breast cancer. Unfortunately, abnormal copies of the genes are often passed down from mother to daughter, and so a woman may be at higher risk of developing breast cancer if it runs in her family. If you have a family history of breast or ovarian cancer, it is important to consider setting up an appointment with your doctor or a genetic counselor to determine if genetic testing is something you should consider.

Mental Health
Many women choose to have reconstruction after mastectomy because it has been shown to improve their quality of life, body image, and mental health. Some women choose not to reconstruct, however, and instead opt for a flat chest or a prosthetic bra. Choosing the path of reconstruction should be done for you alone and not because of anyone else’s expectations or desires. Breast reconstruction can be especially fulfilling for women who have been able to cope well with their cancer and cancer treatment, those without additional diseases or adverse medical conditions, and women with realistic goals for the end result of rebuilding their breasts.

Ultrasound
It is immediately noticeable when a saline implant leaks, but the saltwater solution is harmlessly absorbed by the body. The implant can be replaced through a simple surgical procedure. Because it is often not quickly apparent that a silicone implant has failed, an MRI or an ultrasound must be used to detect this.
MRI
Implants may break or rupture for a variety of reasons. Blunt force or trauma, a surgical error, or even a routine mammogram can compromise the implant’s integrity. It is important to inform your mammogram technician that you have implants in place so she may know the correct pressure to exert so as to avoid rupture. When an implant ruptures, it needs to be surgically removed. Rupture is typically detected through an MRI or via visual observation of deflation. Capsular contracture is another issue to watch out for. As soon as an implant is placed during surgery, a capsule beings forming. This is the body’s natural response to placement of a foreign material. Gradually, this capsule may tighten, causing the implant to appear almost like a ball placed beneath the skin. The contracture typically causes the implant to sit high and tight on the chest, and it will feel hard, look distorted, and seem to protrude. The affected breast may become sensitive or painful as well. Surgical removal or loosening of the capsule may be required.
X-Rays
Mammography
Breast implants, whether located in the sub-muscular or sub-glandular location, can interfere with mammography. Mammography centers are well accustomed to performing mammography on patients who have had breast implants by taking a third view of the breasts.
Computed Tomography
You can NOT have an MRI if you have an expander because the expander contains a metal port that is contraindicated for MRI diagnostics. You may, however, safely have a CT scan with an expander in place.
Chemotherapy
You can receive expansions throughout your chemotherapy. If you complete your expansions prior to chemo completion, however, you will have to wait to have your tissue expander/implant exchange surgery until after chemo (and radiation, if applicable) is complete.
Radiation Therapy
Some cancers may necessitate the use of radiation therapy, which greatly increases the risk of capsular contracture. In these cases, Dr. Kim may recommend an autologous reconstruction, as opposed to implant reconstruction, since natural tissue does not tighten in the same way when exposed to radiation. Sometimes, however, an autologous reconstruction is not possible, in which case Dr. Kim uses a combination of the aforementioned methods in order to loosen the breast capsule after therapy is completed.
Radiation Oncology
Dr. Kim and his breast surgery and radiation oncology colleagues will communicate about the need for radiation after the mastectomy and surrounding the reconstruction. Typically, patients have two options in this scenario. First, they can delay their reconstruction until after both the mastectomy and radiation courses are complete. Second, they can have a tissue expander placed at the time of the mastectomy, and they can undergo tissue expansions for several weeks prior to radiation. Then, they undergo their course of radiation. After this therapy is completed (and they have waited ample time for healing, which is typically six months) they can finish their reconstruction by exchanging the tissue expander for the permanent implant. Or, they can finish their reconstruction with a latissimus or abdominal flap instead of an implant. Patients who choose to finish with an implant may have a firmer reconstructed breast with greater capsular contracture and asymmetry than would be the case with a flap.

Wound Care
Follow your wound care instructions from Dr. Kim, which include cleansing the area and applying topical medications. Avoid light activity until you are cleared to resume your everyday tasks and be sure to keep incisions clean. Try to limit movement in the area to prevent stressing the wound and sutures.

A hysterectomy will not affect Mommy Makeover results. Often, patients coordinate the procedures between their gynecologist and plastic surgeon to avoid having surgeries on two separate occasions.
Mohs Surgery
Skin cancer reconstruction is a valuable part of treating skin cancers, such as melanoma and basal cell carcinoma. Even small tumors can require sizeable skin removal to get rid of all traces of cancer. Dermal fillers will add more volume to tissue, but will not eliminate or reduce excess sagging skin. They deliver temporary results and must be repeated regularly to maintain the desirable effects. Breast Reconstruction

Thigh Lift
The pattern and length of your thigh lift surgery incision depends on the location and amount of excess skin and fat to be removed. The incision can be very small and not extend beyond the groin area, or it may be larger and extend from the groin around the hip. Usually, the larger incision is used when Dr. Kim improves the contours of your outer thigh, not just your inner thigh. The partial rhytidectomy, or mini facelift, is a modified version of the traditional facelift that focuses on the lower third of the face and neck. As you get older, your skin loses elasticity and firmness because the production of the proteins collagen and elastin slows down, causing sagging skin, jowling and lines and wrinkles. Lip Augmentation
Volume loss is another part of the aging process that defines lines and wrinkles and causes hollowing in the cheeks. Injectable dermal fillers can reverse the clock in several ways. Chin Augmentation

Skin Cancer
Skin cancers such as Melanoma, Basal Cell Carcinoma, and Squamous Cell Carcinoma can occur on the face or other parts of the body. Often they are associated with sun damage and fair skin but not in every case. These skin cancers can be directly excised and closed or undergo a process known as Moh’s surgery where precise real-time margins are cleared by the dermatologic surgeon.
Lesions
In cases where the skin tumor returns, Mohs surgery is still the preferred option. In squamous cell carcinoma, the five-year recurrence rate was 3% for Mohs and 8% for excisions. Some forms of skin cancer require additional treatment, such as radiation therapy. Follow-up appointments after Mohs surgery and skin cancer reconstruction are crucial for the early detection of new skin lesions.
Chemical Peels
Many women like to add a few extras to their mommy makeover for an even more dramatic transformation. For example, laser skin treatments and chemical peels revitalize the complexion, and dermal fillers can erase fine lines and add volume to lips.

The best ways to do this include maintaining good skin care and leading an active, healthy daily lifestyle. Sun exposure is a detriment to your face and to the beautifying effects of Botox, so be sure to apply SPF year round while minimizing sun exposure with clothing and hats.
Microdermabrasion
Botox may be combined with other cosmetic skin procedures like chemical peels, microdermabrasion, or other dermal fillers to further improve the aesthetics of your face.
