Smoking Cessation
Sometimes all parts of the spine combine to cause problems. There is usually facet and ligament hypertrophy, protruding, often calcified disk and occasionally instability. This can manifest in stenosis, centrally and lateral, with or without instability. Stenosis is associated with decrease oxygen and nutrients to the nerve, especially when there is increased demand such as walking or exercise. This is called claudication. The facets hypertrophy, and there may be over growth of the facet joints to the lamina. The neural foramina become elongated and narrowed, and if there is disc disease there may be a figure eight or right angle that the nerve root needs to travel in order to exit. There will then be thickening of the nerve roots with chronic conditions leading to peri-neural adhesions and ischemic changes. Over time venous obstruction causes hypoxia with the previous mentioned peri-neural fibrosis and persistent symptoms. In the later stages, patients may develop radicular pain with walking, which may progress to pain while being in the upright position. Patients may also have pain with only standing and this neurogenic claudication is different from vascular claudication, which is increased when the blood flow cannot meet the metabolic demands. Patients with neurogenic claudication are able to ride a bicycle without difficulty because of the flexed position, they are able to walk up an incline or stairs, but not able to walk down the incline or down stairs which extends the spine. These signs and symptoms of neurogenic claudication have been attributable to ischemia of the nerve root. Stenosis is most common at L4-5 and then L3-4. Generally, patients have congenitally narrower canal. A variety of medical conditions cause stenosis, such as calcium pyrophostate crystal deposition, amyloid deposition, and intradural spinal tumors. The treatment for advanced disease is difficult and usually succumbs to surgery. However, if caught early, symptoms may respond to anti-inflammatories, spine stabilization exercises, aquatics and epi-dural steroids. Weight loss once again plays a major role, as does smoking cessation. The

Lesions will often present in extremely difficult locations at the base of the skull. These lesions can be benign or malignant. However, because of there location continued growth will likely lead to high morbidity and mortality. It was once thought that the skull base was inaccessible. However, there have been dramatic improvements in anatomy, microscopy, endoscopy, imaging techniques and surgical techniques. This allows resection and reconstruction of the skull and minimalizes brain retraction and intrusion. As a matter of fact with advanced skull base techniques masses can now be removed or debulked that were once thought to be in remote locations. The skull base approach team is often supplemented with ENT surgeons, opthalmalogical surgeons or plastic surgeons. Prior to any intervention the mass should be discussed with the skull base neurosurgeon.

Muscles themselves are often the source of back and even radicular pain. Unused muscle shrink or atrophy, become tired toward the end of the day, hurt with normal use and cannot support the spine leading to further degeneration of the disk and facet. Initial and long term mandatory treatment consist of low impact, continuous motion exercises. Spine stabilizing exercises are started after two weeks. These are done on a daily basis to help rid the body of this very common aliment. The use of non-sedating muscle relaxants continuously for two weeks, but no longer, will help with the spasms. Longer use makes the muscles dependant and weak leading to further loss of tone and pain. Anti-inflammatory medication may help with pain and muscle irritation. Along with exercise and medication, muscle spasms respond to ice early on in the first two weeks and later to warm moist heat for 30 minutes at a time, 4-5 times per day. The use of heat for greater periods of time can promote muscle tissue swelling and of course worsen the problem. Manipulation, massage, ultrasound and muscle stimulation therapy help the spasms associated with muscle disease.

At times symptoms are not clear, they may be multi-dermatomal, diffuse, not correspond to anatomical findings or be associated with multiple changes on radiographic studies. Radiographic studies only show changes in anatomy, they do not necessarily demonstrate pain. MRI demonstrates multiple changes and many times there are multiples areas of radiographic pathology that do not correspond to symptoms or signs.

However, there are times when the symptoms still do not coincide with the clinical findings or imaging studies. In these limited instances provocative pain tests may be administered. An invasive discogram can tell if a disk is symptomatic. This test should be performed by an experienced physician and include the nature, extent, distribution of pain, the ability of the disk to accept the dye, as well as morphology of the disk on x-ray or CT-scan. The discogram must also show areas of no reproducible pain, unless chemical irritation is wide spread.

Radiation Therapy
Usually if the patient has undertaken the risk to have surgery and the mass if of a type that is best treated with additional measures, the patient should then undergo these additional measures if they can be performed safely. It is only in utilizing all the additional measures that we call help the patient lead the longest and best quality life. Many times the measures include radiation therapy. Radiation therapy may be to the area of the mass or may encompass the entire brain. Sometimes depending on the tumor type, radiation may also encompass the entire spine. The length of radiation therapy as well as the days of radiation therapy will be discussed with the radiation oncologist. Radiation therapy like surgery has risks. Radiation therapy may lead to brain swelling as well as damage to the surrounding brain. The radiation therapy can either be conventional or stereotactic radiosurgery. Focused beam stereotactic radiosurgery is done with either a Gamma knife or linear accelerator. Radiosurgery acts as a focal destructive boost to the area of the mass.

141 individuals with acute whiplash injury and 40 controls were examined and interviewed at the time of injury. They had the amount of movement in their neck measured as well as the pain intensity and neurological deficits. The cervical range of motion at injury was very sensitive at predicting disability after one year 73% and if present was 91% specific for whiplash. If combined with the pain intensity and neurological deficits was 94% accurate and 99% specific. The disability prediction was not influenced by a lawsuit being initiated within a month.

Lesions
Are resected with the assistance of the ENT surgeon. The smaller tumors are resected endoscopically. Endoscopic resection is easier on the patient, usually excludes the need for nasal packing and allows the patient to go home in 1-3 days if appropriate. Pituitary surgery is not always required but may be an alternative to failed medical therapy or a necessity to lesions with symptomatic mass effect.

Attempted must be discussed. There are two options, both which have clinical and literature support. One approach is the repeat microdiscectomy, usually through a larger incision. The patient usually does not improve as fast or as far. The other option is a complete discectomy with fusion. Fusion can be beneficial if the disk space has collapsed and there is significant swelling in the adjacent vertebral bodies. Fusion involves a more extensive post-operative rehabilitation. It takes longer, involves more physical therapy and may involve wearing a brace.
