It is estimated that 15 percent of all adult foot complaints involve plantar fasciitis, the type of heel pain caused by chronic inflammation of the connective tissue extending from the heel bone to the toes. Being overweight and wearing inappropriate footwear are common contributing factors.

"It's unfortunate obese adults get caught up in the vicious cycle of avoiding physical activity due to foot or ankle pain, thereby permitting cardiovascular disease and other life-threatening conditions to worsen as a result," said Dr. James Bartley, a member of the American College of Foot and Ankle Surgeons located in Columbus, GA. "For example, in many cases, chronic heel pain occurs from carrying too much weight. Left untreated, it becomes an impediment to physical activity and meaningful weight loss."

Dr. Schreck is board certified in foot surgery by the American College of Foot and Ankle Surgeons. He is a diplomat of the American Board of Podiatric Surgeons and a Fellow of the American College of Foot and Ankle surgeons (FACFAS). Dr. Schreck attended the University of Northern Colorado and then California College of Podiatric Medicine followed by residencies in general and surgical podiatry. He then completed a year of specialized clinical and surgical training in Pediatric orthopedics at University of California, San Francisco. While there, he held a clinical professorship position within the Pediatric Orthopedic Department. Dr. Schreck has written and published articles for the Journal of Foot and Ankle Surgery (JFAS), Journal of Pediatric Orthopedics (JPO). Podiatry Today, Journal of the American Podiatric Medical Association (JAPMA) as well as a chapter in the book, Pediatrics: Orthopedic Surgical Essentials.

Pinched Nerve
Neuromas and tendonitis are other common foot problems that affect runners. A neuroma is a pinched nerve between the toes that can cause pain, numbness and a burning sensation in the ball of the foot. Overly flexible shoes often are the cause and padding, orthotics, or injections usually are effective. Sometimes surgery is the answer if pain between the toes continues for more than six months.

Bunion Surgery
A study published in the Journal of the American Medical Association reported that bunion surgery is more than 80 percent successful in relieving pain. "In older patients, bunions are very debilitating because these deformities worsen and become more painful over time," said Dr. Bartley. "Given the outstanding success rate of corrective forefoot surgery, there's no reason for any senior to be debilitated by bunions. Further, the adverse health implications of being inactive are far more serious than any risk associated with surgery."

Very often there are some simple approaches we can recommend that will ease the pain and make it possible for you to get started on your fitness walking program in very short order. In other cases we may recommend a surgical procedure to ease the pain of deformities such as bunions or hammertoes, and we'll be able to suggest alternative types of exercise you can do until your feet are ready to take you out on the walking paths again.

Corns
Blisters, Corns, and Calluses: Never pop blisters unless they are larger than a quarter or are painful or swollen. Use a sterile instrument to lance the corner, leave the top as a biological dressing, wash, apply antibiotic ointment, and cover with a Band-aid. Corns and calluses are caused by repeated friction, and should be treated by aseptically trimming the dead skin and eliminating the underlying cause.
Ulcer
Diabetics with Nerve Damage: Neuropathy can cause a loss of feeling in your feet. When this occurs, you lose the ability to feel when your feet have been injured. You can injure yourself and not even be aware of the injury until you notice blood on your sock. An infected foot ulcer may develop if left untreated.
Calluses
Many physicians recommend that patients purchase shoes specifically designed to meet the unique needs of diabetic patients. Diabetics are at an extremely increased risk of developing serious foot disorders. Foot problems and even common diabetic complications can lead to hospitalizations, amputation or other problems. Foot ulcers can be caused by improperly fitted shoe gear that can result in an infection and/or possible amputation. To prevent injury to susceptible and sensitive feet and toes the shoe's toe box should be high and durable. The shoe should be lightweight and preferably seamless to prevent any irritation like calluses, infection or blisters, which left untreated, could develop into more serious problems.

At the first sign of heel pain, Dr. Schreck advises runners to do stretching exercises, wear sturdier shoes, and use arch supports. In some cases, icing and anti-inflammatory drugs, such as ibuprofen, are helpful. Should heel pain continue, custom orthotics, injections, and physical therapy might be required. Surgery normally isn't considered unless heel pain persists for more than a year and conservative treatment has failed to bring relief.

During the annual rite of preparing for the new school year, Dr. Michael Schreck, a local podiatrist who specializes in pediatric podiatry, recommends that parents take five minutes at home to check for signs of possible foot disorders that could prevent active children from participating in sports and other activities.
Achilles Tendonitis
It's not what fashion-conscious women want to hearâ€”another warning about high heels. According to a local foot and ankle surgeon, Dr. Alap Shah pump-style shoes often cause significant pain by irritating a common bony deformity on the back of the heel called 'pump bump.' In many cases, it can lead to bursitis or Achilles tendonitis if left untreated.

Unfortunately many seniors believe it's normal to have pain as you get older, but nothing could be further from the truth," said Bartley. "As a result, they usually don’t seek treatment for painful abnormalities, such as bunions, which are caused by genetic defects – not old age – and can be corrected with surgery and a short recovery period."

Shedding excess pounds helps diabetic patients control their disease, but Bartley noted many who experience foot ulcerations and vascular problems caused by diabetes might think they shouldn't exercise. "Every diabetic patient needs regular foot exams to check for possible sore spots and assess nerve sensation," said Bartley. "And with proper diabetic foot care and the right footwear, most patients can follow an exercise regimen that is safe and appropriate for them."

For those beginning college, heel pain and shin splints can plague freshmen not acclimated to walking long distances across campus to attend classes. "We see students every Fall complaining about pain from walking so much everyday," said Schreck. "Daily stretching and proper walking shoes are recommended, and for those with deformities such as hammertoes, surgery is advised to make walking more comfortable."

Foot Neuroma
Neuromas can develop in different areas of the foot. The most common occurrence is called Morton's neuroma, which develops at the base of the third and fourth toes. Thickening and swelling of the nerve usually is caused by compression and irritation from wearing very narrow shoes or high heels that crunch the toes in the front of the shoe. In addition, neuromas can result from running, racquet sports and other physical activities that exert repetitive stress on the forefoot. A neuroma isn't detectable by x-ray, so the diagnosis is based on symptoms and a physical examination to palpate the nerve or a maneuver that elicits an audible click.
Toenail Problems
Toenail Problems: Ingrown nails can cause inflammation and possible infection and usually are treated by cutting the corner of the nail with sterile clippers. Black toenails happen when a blood blister forms under the nail from trauma, and it's best to let the nail fall off by itself. Fungal toenails are yellow, brown, or black and sometimes are irregularly shaped and thick. They are best treated with oral anti-fungal medications.

Of course, exercise walking is difficult, if not impossible, when our feet hurt. And very often, being overweight has contributed to foot problems such as heel pain (plantar fasciitis) and flat feet. So what's an overweight person to do?
Podiatric Surgery
According to a survey conducted by the American College of Foot and Ankle Surgeons, people over the age of 65 who had foot surgery to correct painful foot problems were very happy with the results. They reported that they were more active than they had been prior to the surgery, and were enjoying social and recreational activities more than they had in the past. In fact, 99 percent of them said that they'd recommend foot surgery to others their age with painful foot problems.
