Immunizations
A couple can try to plan a pregnancy, or try to avoid pregnancy, once there is an understanding of the ovulation (and menstrual) cycle including identification of the most fertile days surrounding the release of a fertilization capable egg. When planning a pregnancy, you should start folic acid (400 micrograms or 0.4 mg a day is recommended to reduce the risk of certain birth defects, which can be obtained in foods fortified with folic acid, vitamins or a folic acid pill), watch what you eat (fruits, vegetables, whole-grains, calcium rich foods are great for you and fatty foods might be avoided), quit cigarettes and alcohol (or drugs), reduce stress in your life when possible, get plenty of sleep and exercise, control any chronic medical problems (like diabetes and asthma), check that your Rubella immunization is still effectively protecting you (blood work can be ordered), and review medications that you take with your doctors.

A different set of medical concerns exists if the woman has “never menstruated” (primary amenorrhea) as opposed to the absence of menses in a woman who has previously had a spontaneous menstrual flow (secondary amenorrhea). Any female who has not had a menstrual flow by the age of 14 in the absence of secondary sexual characteristics (such as breast development) or the age of 16 (regardless of secondary sexual characteristics) should consult a gynecologist or infertility specialist with training in reproductive endocrinology.
Hypothyroidism
This section includes discussions of hypothyroidism (low thyroid function), hyperthyroidism (elevated thyroid function), hyperprolactinemia (elevated circulating prolactin concentration), and ovulation induction using clomiphene citrate (Serophene or Clomid) or the menotropins (Pergonal, Humegon, Metrodin, Fertinex, or the new recombinant forms of FSH). The use of donor eggs for In Vitro Fertilization is also presented.

MRI
Magnetic Resonance Imaging (MRI) of the pituitary gland has undergone enormous development since the initial reports (early 1980s) and has some advantages over Computed Tomographic (CT) scanning (including greater soft tissue contrast and absence of bone artifact). Reportedly, over 75% of pituitary tumors are hormone secreting and the most common hormone secreting pituitary tumor secretes prolactin (hormone).

Cancer and any treatment with radiation or chemotherapy. Available medical reports describing exactly which areas have been irradiated (and the doses) or which chemotherapeutic agents were used (with cumulative doses) are important

Hysterectomy
Amenorrhea may additionally be caused by non-ovulatory problems such as anatomic abnormalities that either do not allow for the growth of the uterine endometrium (such as following an hysterectomy) or do not allow the lining that is shed to exit the body via the vaginal vault (such as with an imperforate hymen).
Laparoscopy
Despite the relative accuracy of these different methods of determining ovulation, this couple has had 1-2 years of "trying" with frequent intercourse in the midcycle without success. In this situation, I would suggest completing the basic assessment including a postcoital test. If the postcoital test is abnormal, then I would suggest several cycles of timed natural cycle intrauterine insemination. If the postcoital test is normal, then the entire basic evaluation is normal and I would consider a pelvic evaluation (laparoscopy and hysteroscopy) to identify and treat pathology in the pelvis.

Laparoscopic Surgery
If the pain recurs every month, persists beyond a few days, or is associated with bleeding into the pelvis and abdomen then more aggressive management may be required. Surgery, with laparoscopy (or laparotomy if facilities are not available to do laparoscopic surgery) are sometimes required to assess (and treat) acute and chronic pelvic pathology.

In girls with precocious puberty, about 75% of the time a cause is not identified. About 10% of the time (female precocious puberty) an ovarian tumor or cyst is found, with almost all of these being estrogen producing "granulosa cell" tumors (although non estrogen producing tumors can also cause early pubertal changes). About 5-10% of the time (female precocious puberty), a head or brain problem is found which may include a variety of lesions (such as abnormal bony skull development, hypothalamic hamartomas that are composed of an abnormal mix of tissues, tumors such as a craniopharyngioma, recent trauma to the skull or brain, infection like encephalitis or meningitis, and hydrocephalus). About 3-5% of the time (female precocious puberty), a disorder called McCune-Albright syndrome is identifiable (including multiple cystic bone lesions, fragile bones that fracture easily, and various size and shape cafe au lait skin spots). Less than 1% of the time (female precocious puberty), adrenal tumors or ectopic production of gonadotropins (by organs other than the pituitary gland) may result in precocious puberty.
