Primary Care
A perplexing urologic problem that I frequently hear Primary Care Physicians ask about is microscopic hematuria. I have decided to devote a PCP Update to this topic acknowledging that the vast majority of the following information was taken right from the American Urologic Association Guidelines for the diagnosis, evaluation, and follow-up of asymptomatic microscopic hematuria in adults.
Smoking Cessation
I have never met anyone who has smoked for several years who declares that they enjoy smoking or that they could quit anytime they wanted. Nothing could be farther from the truth. Studies have shown that nicotine addiction is as hard to break as heroin or cocaine addiction. This article will focus on tips and techniques to help smokers kick the habit. After reading this article and if you are a smoker, you will have suggestions to help get the nicotine monkey off your back.
Sleep Disorders
In patients with documented low testosterone levels, testosterone supplements are used in place of or in addition to a oral medication. Addressing other risk factors for ED, such as removing the patient from medications that cause ED; treating sleep disorders; reducing stress; counseling the patient to limit or quit smoking, alcohol, and recreational drug use; and educating the patient and his partner about the factors that can escalate ED, may help to improve treatment outcomes in addition to the use of oral medication.
Male Infertility
Varicocele may turn out to be the most common cause of male infertility. Fortunately, at this time, it is also the most treatable. Between 30 and 40 percent of all infertile men have a varicocele - a varicose vein of the testicle - as their only symptom. Also, 15% of the male population has varicocele that cause no symptoms and don't appear to cause infertility.

Because of the coexistence of ED and heart disease, it is suggested that all men over the age of 50 who present with ED be evaluated for heart disease, especially in men with other risk factors such as smoking, hypertension, diabetes, hypercholesterolemia, and obesity. It is recommended that a man with ED be screened for heart disease. This usually means an EKG, a few blood tests, and possibly a stress test which consists of running on a tread mill while attached to a heart monitor.
Urinary Incontinence
Stress urinary incontinence refers to the leakage of urine that occurs during physical activities, such as coughing, sneezing, walking and lifting. It is not surprising that many women lose urine during exercise given the impact exercise may have on the bladder, the urethra or the tube that allows urine to exit the body, and the pelvis. If women have urinary incontinence during exercise, it is not uncommon for many women to give up exercising entirely because of the social embarrassment associated with this condition. One-fifth of women who exercised recreationally stopped exercising because of the incontinence.

Life's events can weaken pelvic muscles. Pregnancy, childbirth and being overweight can do it. Luckily, when these muscles get weak, you can help make them strong again. Pelvic floor muscles are just like other muscles. Exercise can make them stronger. Women with bladder control problems can regain control through pelvic muscle exercises, also called Kegel exercises.

Kidney Stones
Kidney stones, one of the most painful of the urologic disorders, are not a product of modern life. Scientists have found evidence of kidney stones in a 7,000-year-old Egyptian mummy. Unfortunately, kidney stones are one of the most common disorders of the urinary tract. In 2000, patients made 2.7 million visits to health care providers and more than 600,000 patients went to emergency rooms for kidney stone problems. Men tend to be affected more frequently than women.
Testicular Cancer
Testicular cancer is the most common type of cancer in men ages 20 to 35. Yet, because it accounts for only about 1 percent of all cancers in men, many people have never heard of this type of cancer. Testicular cancer is of special concern to young men. It can occur anytime after age 15. It is less common in middle-aged and older men. White men are four times more likely to develop testicular cancer than black men. The rate among hispanic men lies between those of blacks and whites. Two groups of men have a greater risk of developing testicular cancer--those whose testicles have not descended into the scrotum and those whose testicles descended after age 6. Testicular cancer is 3 to 17 times more likely to develop in these men.
Cervical Cancer
You could look at these numbers and figure guys get off lucky. What I see, however, is responsibility. If you fool around and get infected, the odds of you infecting your spouse are very high, with potentially devastating effects. Either of you could become sterile, and your partner may suffer from chronic severe pain or develop cervical cancer. The days are over when sexually transmitted diseases were minor health problems that could be cured with a dose of antibiotics. Only two of the eight on this list respond readily to medication, and even those are developing resistance. Today, the old saying that a one-night stand can last a lifetime has more than one meaning.
Interstitial Cystitis
Abstract: Although it's not a malignant or infectious condition, interstitial cystitis can be as disabling and sometimes even more difficult to manage. Many women undergo ineffective gynecologic surgery, and some men have inappropriate transurethral prostatectomy in a desperate attempt to subdue their pain. The authors provide tips for primary care physicians on making a presumptive diagnosis and suggest symptomatic relief methods to try before referral.
Urinary Tract Infection
A separate article will discuss gross (visible) hematuria which does require urologic evaluation unless secondary to a documented urinary tract infection by culture.
Prostate Cancer
Each year nearly 250,000 men will be diagnosed with prostate cancer. The disease will cause nearly 40,000 deaths. Most men with prostate cancer can be helped. This article will review the function of the prostate gland, the causes of prostate cancer and how the diagnosis of prostate cancer is made.

Damage to the cavernous nerves that originate in the pelvis can inhibit the initiation of sufficient stimuli to achieve an erection. Such damage may result from lower spinal cord injury and from non-nerve-sparing radical prostatectomy. Low testosterone levels -- which are usually responsible for a decrease in libido -- are frequently associated with aging and can cause ED. Abnormal thyroid hormone levels (both hypothyroidism and hyperthyroidism) and elevated prolactin levels are also associated with an increased incidence of ED. Endocrine disorders other than diabetes -- such as hypogonadism, thyroid disease, and hyperprolactinemia -- are a less common cause of ED; however, they can be readily treated

Ultrasound
It appears we are doing everything to the prostate. Microwave, ultrasound, radio frequency waves, laser, and even injections of alcohol! Almost all of these can be done as an office procedure with local anesthesia, supplemented by oral medication. Most require a catheter for one or two days and can be associated with worsening of the symptoms prior to improvement.
MRI
When placed in the strong magnetic field of an MRI scanner, the protons of hydrogen atoms in the prostate tissue align with the magnetic field. Radio frequency pulses applied to that field produce images that the radiologist can review when the protons are displaced by this alignment.

For example: Disclosures of medical information about you may be made to doctors, nurses, technicians, or others who are involved in taking care of you at a Texas Health Physicians Group practice. This information may be disclosed to other physicians who are treating you or to other healthcare facilities involved in your care. Information may be shared with pharmacies, laboratories or radiology centers for the coordination of different treatments.

Services such as biopsy, CT scan, laparoscope, robotics, ultrasound and urodynamics may be used in diagnosing and developing a comprehensive, individualized treatment plan for the urology patient.
Chemotherapy
No one read the newspapers the last week in July and couldn't help but notice and admire the courage of Lance Armstrong, the 1999 Tour de France winner of the most prestigious race in bicycling. Lance Armstrong had cancer of the testicle that spread to his lungs and brain when it was diagnosed. Lance Armstrong was not in just a bike race but in a race for his life. He made a comeback after multiple surgeries and several courses of chemotherapy that was considered impossible by nearly everyone except Lance Armstrong!
Radiation Therapy
There are treatments for all patients with cancer of the prostate. Three kinds of treatment are commonly used: surgery (taking out the cancer cells), radiation therapy (using high-dose x-rays or other high-energy rays to kill cancer cells) and hormone therapy (using hormones to stop cancer cells from growing).

Dr. John Fairbanks is Board-certified in Urology. Dr. Fairbanks completed his residency in General Surgery, in the Department of Surgery, at the University of Cincinnati in Cincinnati. Dr. Fairbanks completed his residency in Urology and also served as Chief Resident in the Department of Urology at the University of Cincinnati. He completed his Doctor of Medicine at the Chicago Medical School. Vasectomy
Vasectomy is the process of dividing the vas (the tube that delivers the sperm from the testis to the prostate) in order to prevent conception. It is the most common method of male contraception in this country where about 500,000 vasectomies are done each year. Since vasectomy simply interrupts the delivery of the sperm, it does not change the hormonal function of the testis and sexual drive and ability remain intact. Since most of the semen is composed of fluid from the prostate, the semen will look the same. Vasectomy is thought to be free of known long term side effects, and is considered to be the safest and most reliable method of permanent male sterilization.

Chancre: A hard, syphilitic primary ulcer, the first sign of syphilis, appearing approx. 2 to 3 weeks after infection. The ulcer begins as a painless lesion or papule that ulcerates. Occurs generally singly, but sometimes may be multiple.
Lesions
Recently, you underwent a prostate biopsy because of a suspicion that you have a prostate malignancy. Over the past few years we have learned a great deal about the various types of prostate problems that may occur. Saying that prostate biopsies are either benign or malignant is a simplification of a very difficult and complex task. Prostate tissue can present with many different findings and the pathologist's job to interpret these findings is difficult in many cases. Recently, we have been able to determine that premalignant prostate lesions exist. This means that cancer is NOT present, but the changes seen under the microscope suggest that cancers MAY develop later on. We have come to call these findings 'prostatic intra-epithelial neoplasia' or 'PIN.' The pathologists will rate or grade PIN lesions from 1 to 3 with 1 being only slightly unusual and grade 3 being very unusual and very close to being called cancer or malignant. More recently, PIN has been reclassified as Low grade and High grade, with PIN 1 being Low grade and PIN 2 & 3 being High Grade.
