Pregnancy
Labor and Delivery
Many women are self-conscious about the appearance of their labia due to aging, weight loss, or childbirth. An unattractive bulge in swimwear or bike shorts is the main concern for many; others dislike intimate encounters because they feel their genitals are abnormal. For most, improvement is straightforward. Both labia majora and labia minora can be improved; the outer lips can be tightened with radiofrequency or surgically reduced. The labia minora, or inner aspect, usually requires minor surgery to reduce or contour.

Urinary Tract Infection
Capsular contracture occurs in almost a third of women undergoing breast implant surgery. This can happen when there is bleeding into the pocket, or if inflammation occurs. In some cases, a sudden, delayed-onset contracture can occur with a serious local or systemic infection, such as a dental abscess or urinary tract infection. The 52-year-old woman below had a hematoma due to trauma and developed a very tight pocket on the left.

Ultrasound
A traditional face and neck lift is needed if you have significant skin laxity. The neck can be approached by itself, but generally tightening the jawline is indicated as well. Short incisions can be used in many cases. Dr. Duncan is very careful to avoid distortion; your face will not look odd or “done”. Recovery time is about 7-10 days. RF assisted liposuction utilizes the Inmode FaceTite device or J Plasma, or a combination. It is chosen if skin laxity is mild to moderate, and the amount of skin tightening needed is less than 30%. In this procedure, tumescent fluid is infused and some liposuction is performed, if indicated. A compression garment is worn at night for several weeks, but patients can return to work within a couple of days. Laser and fat grafting are a good alternative if skin quality and volume loss are a concern. This combination costs less than a traditional facelift and can help take years off your aged appearance. Recovery for laser resurfacing and fat grafting is about a week. Minor procedures that can help include filler injections, or nanofat plus Allofill, if volume loss is a main concern. Resurfacing with the fractional laser, Halo, RF needling, or microneedling are good first steps if the idea of surgery is not for you. Soft tissue tightening with Thermismooth or Forma can add firmness and tone to aging lax regions. Lumecca and BBL are broad band lasers that reduce hyperpigmentation and prominent blood vessels. Rosacea can be well treated with Halo, based on recent research using high resolution ultrasound.

She completed her Bachelor of Health Sciences with a Biochemistry Minor in the Dual-Degree Entry Level Masters Physician Assistant Program at Quinnipiac University in 2019. She continued at Quinnipiac University where she completed her Master of Health Sciences in Physician Assistant Studies in 2021. With her profound interest in surgery, she decided to further her training in the Norwalk Hospital/Yale School of Medicine PA Surgical Residency Program. During her rigorous training, she had the opportunity to work in various surgical specialties including general surgery, plastic and reconstructive surgery, neurosurgery, cardiac surgery, colorectal surgery, surgical ICU, pediatric surgery, bariatric surgery, and vascular surgery. With a profound interest in plastic surgery, she chose to complete an additional elective in this field of medicine during her residency training. Brow Lift
This 28-year-old had a sad and anxious expression since her early teens, due to the position of her eyebrows. Tight attachments to the temporal fascia and low outer brows made this appearance hard to disguise. She had a temporal brow lift with a small incision within the hairline to correct this concern. Neck Lift
Occasionally a single procedure can dramatically improve facial appearance, especially if a strong history of sun exposure is the cause of aging. Many of our patients start reversing their apparent age with full field erbium laser resurfacing. Wrinkle reduction, correction of pigmented spots, improvement of acne scarring, and restoration of luminosity to dull, dry skin are hallmarks of this procedure. If there is volume loss or significant skin laxity, laser treatment can be safely combined with either fat grafting or fillers, plus a mini-neck lift or RF neck lift.

The first “fix” we consider for protruding ears is an otoplasty. Ears that stick out are a common source of teasing and self-consciousness in children and teenagers. Other concerns patients have regarding their ears include elongated earlobes, earring hole tears, or earlobe distortion due to gauge type earrings. In many cases, gauge deformities can cause a person to be rejected from a job or military service. Correction of these hard to hide concerns is usually straightforward. In many cases, the procedure can be performed in the office under local anesthesia.
Body Contouring
While nonsurgical procedures can achieve good results, a very visible difference is more likely to be seen with a minimally invasive approach. Most middle-aged women seek improvement in the regions around the eyes and mouth. If you don’t have much time before your event, a Halo laser treatment along with Botox and fillers might be best. If you do have a bit more recovery time, the best facial options might be resurfacing and fat grafting, with radiofrequency-assisted skin tightening. Body contouring is usually minimally invasive, so you would be able to attend your reunion in a few weeks.

Patients come to us for a variety of reasons when seeking laser resurfacing. Many times our patients simply want to look better for themselves. Sometimes they are seeking out a new relationship, and increased confidence is a must. For example, acne scarring reduction is addressable with laser resurfacing. We have clients where a more youthful look allows them to continue their careers. A rejuvenated look makes many of our patients feel younger, not just look like it! On the other hand, some patients look to reduce their skin cancer risk. Laser can significantly reduce sun damage.

There are many aesthetic uses for SYLFIRM X, but I especially like to treat aging skin, skin laxity, pigment, and vascular lesions. I have also seen excellent results treating acne scarring, rosacea, facial pigment, fine lines, and other lesions.
Chemical Peels
We offer quite a few noninvasive treatments such as IPL, BBL, and Lumecca for treating sun-damaged skin. Chemical peels can help with pigment, tone, and texture. Microneedling is a very popular new treatment that can rejuvenate the skin with little to no downtime. External radiofrequency treatments can add tone to lax tissue.
Skin Care
The effect of a full surface (not fractional) treatment can last up to 20 years if maintained by regularly using a good sunblock and a good skin care regimen. The procedure is safe to perform on an annual basis if a more modest procedure is performed. A study performed in California showed a reduction of genetic age markers following Sciton laser treatments. While laser resurfacing will not prevent aging, it can reverse the appearance of pigmentation, wrinkles, solar elastosis, acne scarring, and crepey skin.
