G & G Living Centers provide residential services include six ICF/MR group home programs and eight HCBS-Waiver home services. A total of 60 individuals currently serve through these programs. Services provided through both residential programs include transportation to places of employment, to medical and dental appointments and for personal shopping, an outcome focused environment, development of programming including Individual, comprehensive Plan (ICP), Individual Habilitation Plan (IHP) and Vocational plan. These ensure the provision of active treatment, the preservation of resident rights, and the fulfillment of clinical, functional and personal outcomes. There is also a case management service, including assessments, service plan, service management and service monitoring. A day activity center is available for those persons who currently have difficulty with a full workday. The majority of individuals who choose skills training at the day activity center are limited by severe or profound disabilities. Vocational, life skills, and community based assessments and evaluations are available to any person or referral source. Technical assistance, vocational, career planning and life skills, consulting services, community job settings, and volunteer opportunities are available. G & G Living Centers are located in Guttenberg, Iowa.
