And, within these elegant surroundings, you will find short-term as well as long-term health care and rehabilitation services that are second to none. Summerfield Health and Rehabilitation Center offers both private and semi-private rooms, all Medicare approved and each with a window to a lovely view. Our interdisciplinary team for transitional care brings the skills of a variety of specialists to work toward your recovery. You get the benefit of their knowledge in developing and implementing a personalized care plan designed to speed your recovery and keep everyone involved during the transition process. Weekly updates are provided by the social services director so that you will always be informed.

        By far the most disgusting, unorganied facility i have eer encountered. I live in Minneapolis and my grandmother suffered a stroke last week and was discharged to this facility following her hospital stay. me, my mother nad my uncle flew from Minnesota to tend to my grandmother. My uncle and mother are my grandmothers power of attorney and upon arriving at th hosipital, they signed all of the neccary paperwork. The next day, a nurse came and asked for my mother to sign the same paperwork AGAIN because the staf lost it. my mom refused and informed them they already signed the fors and that they have copies. the staff insisted that they sign again.. my mom didnt. My grandmother has reported REPEATED unanswered call lights. They LOST her medication order from the doctor. They were supposed to install a heart monitor.. it was never installed. They gave her food on severeal occasions without providing water. CHOKING HAZARD!! Th food was under cooked and inedible. CONT>>>
