Care One at Madison Avenue

This facility is so poorly run that most everything goes wrong. The staff turnover rate is astronomical. During a 5 weeks stay, there were three different Social Workers. Therefore, nothing at all was done correctly by that department. The head nurse left and they appeared to be looking for someone to train for that position. The physical and occupational therapists were different people almost every day. Sometimes, they were simply supplied by an agency and knew nothing at all about each resident. Once, my Dad received therapy for a hip replacement, when he had not had a hip replacement at all. It was totally appalling to realize the lack of communication in this place. I would recommend you never allow anyone to come to Care One at Madison Avenue for any reason.
