Endoscopy
In 1991, an endoscopy found a tumor on my pancreas, and the prognosis was dire. I was determined to pursue every avenue open to me and thus arranged for an ultrasonic endoscopy by Dr. Snady. Dr. Snady's skill with the EUS was able to determine with precision and accuracy the exact position and size of the tumor. Endoscopy is painless and takes a brief period of time in his office. Dr. Snady was instrumental in introducing me to the oncologist and other medical practitioners and to a course of treatment, which has enabled me to survive to this day.
Prostate Cancer
Because I was diagnosed as having prostate cancer and had a radical prostatectomy in January, 1998, Lorraine's treatment was extended through January and February to March, 1998. Dr. Bruckner decided that if I had surgery first then I would be able to better care for Lorraine after her surgery.
Lung Cancer
In patients with lung cancer EUS has proven very useful in determining whether lymph nodes in the chest are malignant or benign. This is determined mainly by performing fine needle aspiration (FNA) of these lymph nodes with EUS guidance. Two other methods of obtaining biopsies of these lymph nodes are mediastinoscopy and thoracoscopy. However, these must be done in the operating room and are therefore riskier.

Ultrasound
If you have been told that you have or might have cancer based on the results of tests such as standard sonogram/ultrasound, CT scan, MRI, endoscopy, ERCP, biopsy, blood tests, etc, confirming the diagnosis becomes the most important next step. Then, if it is cancer, determining the exact extent of the malignancy (staging) is essential to getting the best treatment. Sometimes a mass or tumor is found which may look like a cancer, but is not. Masses can sometimes be caused by inflammation that may clear up on its own.
MRI
Cancer of the pancreas frequently does not cause symptoms until it is advanced. CT scan, angiogram and even MRI will frequently miss tumors less than two centimeters in diameter (more than one inch). EUS can find tumors that are smaller than three millimeters (less than a quarter of an inch).
X-Rays
Computed Tomography
Stones can also occur in the bile duct or pancreatic duct. Stones in these locations can usually be diagnosed with standard blood tests and an accurate patient history. A standard sonogram is often misleading and usually not helpful. The presence of stones in the ducts are then confirmed and treated using a side viewing endoscope for a technique called therapeutic ERCP (Endoscopic Retrograde CholangioPancreatography). During ERCP dye is injected into the bile duct and pancreatic duct. Stones can then be seen with x-ray and then removed through the side viewing endoscope. However, ERCP does have at least a 5% complication rate associated with it, although most complications are not major or severe. Therefore, ERCP is best used when there is a high probability of performing a therapeutic technique, since EUS has an extremely low complication rate and is as accurate in finding stones. If the diagnosis is in doubt, EUS can be used as a alternative to ERCP to determine the best treatment plan. A new method, MRCP (Magnetic Resonance CholangioPancreatography), is better than standard sonography or CT scan to look for stones. But, MRCP is not correct often enough that it is best used when stones are more likely not present. These days, the treatment of stones in any location rarely requires an open operation.

I then went to see Dr. Howard Bruckner for chemotherapy and Dr. Jack Dalton for radiation treatment at Mt.Sinai Hospital, who began the fight against this cancer, to win back my life.

Cyst
Ulcer
On April 16, 1997, Lorraine was diagnosed and treated for a duodenal ulcer here in New Jersey by gastroenterologist Dr. Thomas Fiest. Our family physician, Dr. Kenneth Granet, did follow-up medical checks, blood work and other testing. On July 11, 1997, during a follow-up examination, Dr. Granet noticed Lorraine had a jaundiced, yellow complexion. As a result, he had her admitted to a local hospital for stabilization and diagnoses. During that time, she was diagnosed as having pancreatic cancer.
Lesions
Parts of the liver can be seen, quite well when EUS is used in the stomach and small bowel. EUS can therefore be used to investigate whether there are any focal tumors, cysts or scar tissue, which may require EUS guided FNA. Because EUS can find lesions that are less than 1 to 2 cm, it can find metastatic tumors that cannot be seen on other imaging tests such as CT or MRI.
