Primary Care
Our staff will make every effort to simplify all billing and insurance claim procedures. We participate with many insurance plans; however, we recommend you call your insurance company prior to your visit to verify coverage and to determine if a referral from your primary care physician is necessary. Please bring your insurance card with you to every visit so that we may ensure that our records are accurate. If you do not have insurance coverage, full payment is required at the time of service.

Blake’s goal is to let patients know that behavioral health concerns are more common than we may realize, and there are many great options for management and treatment. As a nurse, Blake has seen the need for behavioral support in the community firsthand, and hence he felt like he could connect on that level when pursuing his career in Psychiatry.
When you get services from an in-network hospital or ambulatory surgical center, certain providers there may be out-of-network. In these cases, the most those providers can bill you is your plan's in-network cost-sharing amount. This applies to emergency medicine, anesthesia, pathology, radiology, laboratory, neonatology, assistant surgeon, hospitalist, or intensivist services. These providers can't balance bill you and may not ask you to give up your protections not to be balance billed.
