The electroencephalogram, also called the EEG, or the “brain wave test” was invented in 1929 and is therefore one of Neurology’s oldest tests. Although it was once used to help diagnose a wide variety of neurologic problems, the EEG, like the lumbar puncture, has come to have a fairly limited usefulness. This usefulness is primarily in helping neurologists determine the cause of events commonly referred to as “spells.” Neurologic problems come in a variety of forms, and one of the more common forms is that of a transient and involuntary alteration in body position, thought content, or consciousness. In other words, “spells.” The electroencephalogram may help diagnose different kinds of seizure problems (epilepsy), feinting disorders (syncope), or sleep disorders (narcolepsy).

Emergency Care
If you call the clinic with a question, a request, or a matter for discussion, we will call you back the same day, usually later in the day after we have seen our scheduled office patients. If the matter is something that needs immediate discussion we will call you back as soon as possible. True emergencies should bypass our office and be attended to in an urgent manner, either by calling 911 or by taking the patient to an emergency room.

East Portland Neurology is located at 10101 SE Main Street, Suite 2004, on the second floor of Building 2 at the Adventist Medical Center, near the junction of Interstate 205 and 84 in Portland, Oregon.
Carpal Tunnel Syndrome
Diseases that a neurologist might commonly see include headaches, epilepsy and other disorders that cause alterations in awareness, Alzheimer’s Disease and other conditions affecting the mind, Parkinson’s Disease, Multiple Sclerosis, diseases that effect peripheral nerves such as Carpal Tunnel Syndrome and peripheral neuropathies, conditions that cause pain, and many other disorders that interfere with strength, sensation or movement. One of the more common problems that neurologists see and care for is stroke. Although all three of our neurologists see those affected by stroke, Dr. Howard Taylor has a special interest in stroke and manages Adventist Medical Center’s Stroke Unit.
Multiple Sclerosis
Though once used frequently to help diagnose a variety of neurologic problems, this test, also known as an “LP,” a “spinal tap,” a just a “tap,” is now used infrequently. Many years ago when there were only a few neurologic tests available, the lumbar puncture was used much more commonly than it is today. Now there are many different types of nervous system tests in use and as a result the LP is used in only a handful of situations. Today, the LP can help diagnose Multiple Sclerosis, meningitis, and a form of infectious neuritis called the Guillian-Barre Syndrome. Occasionally it is used to confirm one type of bleeding inside the head. A “spinal tap” is also the means by which dye is introduced into the spine for two tests called myelography and cisternography.
Epilepsy
Our Neurologists diagnose disorders using a variety of techniques. A visit begins with a comprehensive history of the problem and a careful neurologic exam. Sometimes the patient may be advised to have one or more tests to further clarify the nature of their problem. There are, of course, a huge number of other tests that might be indicated, but the tests that a neurologist is most likely to order are blood tests, computerized tomography scans (CTs) of the brain, magnetic resonance imaging studies (MRIs) of the brain and spine, and magnetic resonance angiograms (MRAs) of blood vessels in the head and neck. Our doctors are specially trained to perform electrical tests of the muscles ( Electromyography or EMG ) and nerves ( Nerve Conduction Studies or NCS ). These are thought of as an extension of the neurological exam and, if indicated, are sometimes done as part of your first visit. An electrical test of the brain ( Electroencephalography, also known as an EEG or “brain wave test” ) measures brain wave activity and is used to diagnose epilepsy. It is performed in our office by an EEG technician and then interpreted by one of our doctors. Most Lumbar Punctures are performed by our doctors in our office or in the hospital short stay unit and are utilized to diagnose some neurological diseases including multiple sclerosis. Because diseases in other bodily systems can effect the nervous system, it is sometimes necessary to refer a patient to another specialist for additional information. If a second opinion is needed we will confer with our neurology subspecialty colleagues at OHSU.

Psychiatry
Neurologists are medical specialists who diagnose and/or care for diseases and disorders that affect the nervous system: the brain, the spinal cord, and the peripheral nerves. Neurologists do not perform surgery on the nervous system. When surgery is necessary it is done by neurosurgeons. Although there is a kinship between the fields of Neurology and Psychiatry---Sigmund Freud, the founder of Psychiatry, was originally a neurologist---neurologists are not primarily concerned with treating emotional disorders.

MRI
An MRI is an advanced way of taking pictures of the inner brain. It is harmless and involves magnetic fields and radio waves. It is performed when a patient is lying in a small chamber for about 30 minutes. It is painless, but may be stressful for individuals with claustrophobia (fear of closed areas). Our physicians can offer options to help you relax.
Radiology
This test is usually simple and straightforward, especially in a young, relatively thin individual. Obesity or previous back surgery can make an LP more difficult. LPs are usually done in the “short stay” portion of the hospital, but if your neurologist thinks that your lumbar puncture might be difficult to perform, in order to minimize your discomfort he may ask that it be done in the Radiology Department, by a radiologist and under fluoroscopy.
