10. What are the alternatives to Surgery? a. Mohs surgery is not appropriate for all types of skin cancer. There are many different ways of treating skin cancers. The decision to use Mohs depends on a number of factors relating to the cancer, its location, patient factors and prior treatments used. Some alternatives include radiation, chemotherapy creams, scraping and burning, freezing, and traditional excision. We will discuss other options during the informed consent process. Your doctor has referred you for Mohs Surgery as they feel that this is the most appropriate method of treatment for the type of tumor you have. If there is an alternative treatment that may be more appropriate, I will discuss this with you.

Review expectations for the day of surgery, including the likely surgical reconstruction, wound care and bandaging, and activity modification.
Mohs Surgery
Since Mohs surgery requires highly trained personnel, and can be time consuming, it is reserved only for certain cases. The four most common indications for using the Mohs technique are: 1) when the tumor is located on a structure that is so important that one wishes to remove only the diseased tissue and preserve as much of the normal skin as possible (face, ear, hand, etc); 2) when the cancer has been previously treated and has come back (recurred); 3) when the margin or extent of the tumor cannot be identified; 4) when the tumor occurs in an area of the body where it is not effectively curable with other methods.

Dermabrasion
As with all forms of surgery, a scar will remain after the skin cancer is removed and the surgical area has completely healed. Mohs micrographic surgery, however, will leave one of the smallest possible surgical defects and resultant scars. Scars do have the ability, through the body's own natural healing properties, to remodel and improve in appearance for a six to twelve month period. There are also many other techniques available for enhancement of the surgical area following skin cancer surgery. Depressed or indented scars may be elevated. Likewise, a raised or roughened scar may be smoothed, using dermabrasion techniques. Skin flaps and grafts also may require a subsequent "touch up" procedure, to further improve their appearance.

Dr. LaDuca has written book chapters for the Fitzpatrick's Dermatology In General Medicine text and has contributed to numerous medical publications. His clinical interests include psoriasis, acne, and skin cancer, as well as cosmetic dermatology. Dr. LaDuca shares his time between three practices: Reflections Dermatology and Psoriasis Center in Auburn, Skaneateles and East Syracuse.

5. Will my cancer become a melanoma? a. Basal cell carcinoma, Squamous cell carcinoma and Melanoma are all completely different types of cancer. One does not become the other. Each of them has early stages and more advanced stages of the disease, but they are still their own cancers.

Dr. Jeffrey R. LaDuca is a board certified dermatologist and has been practicing in central New York since 2002. Dr. LaDuca received a bachelors of science in biology from Lemoyne College (1989), a Ph.D. in physiology from SUNY Buffalo, Roswell Park (1994), and his M.D. from the SUNY Health and Science Center, Syracuse, NY (1998). He completed his internship at SUNY health science Center in Syracuse, and his residency in dermatology at Strong Memorial Hospital in Rochester. Dr. LaDuca is a fellow of the American Academy of Dermatology. He is also a member of the state of NY medical society as well as the Cayuga and Onondaga medical societies. His teaching experience includes clinical instructor of Dermatology at the University of Rochester and he has lectured extensively to medical groups including physicians, nurses, nurse practitioners, and physician assistants.
