Our Family/General Practice services offers primary care for individuals and families across all ages, genders, and diseases. We provide care for all members of a family during all phases of life - infancy, adolescence, adulthood, and senior years.
Family Practice
Family Health Care Clinic accepts all patients, regardless of their insurance status or ability to pay. We are a Federally Qualified Health Center (FQHC), which means we can offer a wide variety of services through the Sliding Fee Discount.
At Family Health Care Clinic, we recognize that women have many unique health care needs. We have an outstanding team of physicians and nurses, who are dedicated to making every woman feel confident and comfortable in their care. We specialize in pregnancy and childbirth, and provide exceptional care for women of all ages. Knowledgeable of the latest research and technological advances, we offer an experienced, perceptive team focused on the care of each individual woman.
Our Internists look at your body in its entirety and are specialists in diagnostics, long-term care, preventative medicine and more. They are often best able to manage the treatment of ongoing illnesses as well as prescribing preventative measures. With this kind of expertise many patients often see an internist as their primary physician.
Your team may include a doctor, a mid-level provider (physician assistant or nurse practitioner), a medical assistant, a health educator, a behavioral health professional, a dietitian, and other health professionals such as a care coordinator or “patient navigator†to help you find your way through the health system. They will help you get healthy, stay healthy, and get the care and services that are right for you. They will assist you with self-management tools and educational materials. You will be an active participant in all aspects of your care, including the ability to choose your primary care provider who is supported by the Health Home Team. When needed, your Health Home Team arranges for appropriate care with other qualified clinicians and settings such as referrals, consultations, x-rays, and hospitalizations.
